AFFIDAVIT

Service No. 15396337H, SEP, Pawan Kumar aged 48 years residing at Vill. Panolu , Post Office Barswan,
Teh. Balh, Distt. Mandi, State Himachal Pradesh, Pin-175036 hereby solemnly affirm and swear as
follows:

I am serving with 631 DSC PL of Defence Security Corps. My daughter's name has been Deepti Kumari in
her Birth Certificate issued from Department of Health, Netaji Subhash Chandra Bose Zonal Hospital,
Mandi, Distt. Mandi, her Aadhar Card bearing No. 7913 0105 7444 and in her School records. But service
records, Part II Order No. 0/0139/0005/2003 dt. and DSC Record, Part II Order No.
0/0039/0011/2023 dt. 29.05.2023, my daughter’s name has been entered as Deepty Devi, in fact the
correct name of my daughter is Deepti Kumari. I swear that both Deepty Devi and Deepti Kumari are one
and the same person.

I am swearing this affidavit for the purpose of changing the name of my daughter in my documents as
Deepti Kumari instead of Deepty Devi.

Facts stated above are true and correct.
