MEMORANDUM OF AGREEMENT
BETWEEN THE BARANGAY NUTRITION SCHOLAR – CITY MAYOR
KNOW ALL MEN BY THESE PRESENTS:
This agreement is entered into by and between Fe B. Joseph, legal age,

Trained as BARANGAY NUTRITION SCHOLAR to be deployed in Barangay Silop and

Hon. Pablo Yves L. Dumlao II Chairman of the City Nutrition Committee of SURIGAO CITY.

WITNESSETH
That FE B. JOSEPH shall render services to BARANGAY SILOPfor one year,
Name of Barangay
Starting JANUARY to DECEMBER 2024.
That during his/her service year, he/she shall perform the following tasks:
1. Monitor the height and weight status of children in his/her barangay.
2. Deliver basic nutrition and related health services in the absence of other barangay-based community
workers who can provide the same services.
3. Facilitate / coordinate the delivery of services to his clientele by other community based workers.
4. Deliver the capabilities of the community to be self-reliant in solving their identified malnutrition
problems.
5. Record, monitor and report services rendered to the nutritionally-at risk families in the barangay.
That the City Nutrition Action Officer and the City Nutrition Program Coordinator shall be the direct
supervisors of the said BNS and shall be thus responsible in assessing performance, and;
That, the local government through the City Mayor shall give financial and moral support to the BNS for
the duration of her service year; and thus help achieve the goals of the Philippine Plan of Action for Nutrition.
I witnessed thereof the parties have hereunto affixed their signatures, this__day of month & year
