10 Whether oxygen saturation level (SPO2) through pulse oximeter of suspected case is maintained
Whether face shield issued to all workers and staff working in proximity (closely work due to

safety requirement)
Whether sufficient no's full body PPE suit are available to prevent infection during medical
12
emergency
Whether there are sufficient nos. of wash basins, liquid soap with running water to maintain
13
hand hygiene of workers and staff
Whether there are sufficient nos. of hand sanitizer dispensing points created inside the site citing
14
pandemic requirement
Whether all available workers, staff & visitor submitting their relevant information in Form 1 (Self
15
Declaration)
Whether sufficient quantities of personal and area sanitization/disinfection material such as
16
(Hand sanitizer, Sodium hypochlorite / QAC, Sprayer etc.) are available
Whether there is any team or worker designated / assigned for upkeep site and guesthouse
17
sanitization
Whether an isolation facility (to handle any emergency) available on the site by the Client / S&W
18
(Emergency Nos with nearby COVID Hospital)
Whether all waste CPE (nose mask) and bio-medical waste generated disposed off in regular

intervals
Whether there are safe and practicable measures or process adopted to maintain social

distancing at Site & Guesthouse
Whether separate / dedicated facility for dining area available inside the site premises by the
21

Whether the social distancing norms following in commutation from guesthouse to site (To and
