Dear sir,
I am John Brent V. Molina, a student of Bachelor in Secondary Education Major in English. I am
writing this letter to ask for your approval of my enrollment. I couldn’t enroll in my internship
subject because of my INC in Field Study 2 (FS2). I was able to comply and complete all the
requirements of my subject and I already have grades in it.
The submission of my portfolio was on August 21, 2024, and was recognized as late. I was able
to obtain the signatures of Ma'am Verallo and Dr. Sebial for my INC form, which was then fully
processed on August 27, resulting in a grade change to 2.25. Since then, my enrollment is still in
process until today.
I want to offer my sincerest apology for my behavior and I have learned a lot of lessons from it.
Despite all that happened, because of my negligence, I still want to continue my education and
graduate on time. I am hoping that you will give me this chance to improve and continue my
journey to reach my dream.

Dear sir,
I am Christian Joshua H. Matulac, a student of Bachelor in Secondary Education Major in
English. I am writing this letter to ask for your approval of my enrollment. I couldn’t enroll
in my internship subject because of my INC in Field Study 2 (FS2). I was able to comply and
complete all the requirements of my subject and I already have grades in it.
On July 26, 2024, I submitted my portfolio to address a missing grade, but unfortunately, it seems
to have gone unnoticed. On August 16, 2024, Ma'am Gejazhylle P. Engaño informed me that I
could not proceed with enrollment due to an INC grade in Field Study 2. As soon as I became
aware of the issue, I reached out to Ma'am Jezreel V. Verallo, who confirmed my portfolio
submission, and I immediately began the process to resolve the INC grade.
On August 21, 2024, I had obtained the necessary signatures from Ma'am Verallo and Dr. Sebial
for the INC form. However, the process experienced delays due to the foundation day celebration.
I deeply apologize for any inconvenience this may have caused. Thankfully, the school registrar
accepted my INC form, and on August 27, 2024, my grade was updated to 2.00. since then my
enfrollment is still on process until today
I want to offer my sincerest apology for my behavior and I have learned a lot of lessons from
