Patient Details
• 56 years old, Female,private
• The patient has the following medical conditions: Diabetes (Type 1)
• The patient takes the following medication: Insulin
• The patient does not have allergy No latex allergy
• The patient does not smoke.
• The patient drinks two units of alcohol/week.
• The patient´s last dental visit 6 months ago
Preparation of the clinical area
As I started my shift in the morning I have switched on all the light.
I washed my hands following HTM 01-05 guidelines and dried them with soft paper towels. I
changed into my clean uniform and appropriate shoes and washed my hands and dried them
with paper towels. I put on my personal protective equipment (PPE) such as facemask, gloves
( single use),visor and Plastic apron I switched on all equipment and checked if they work
correctly this included compressor, suction, main switch in surgery and the lights, chair,
amalgamator (and the tap water from sinks for 30 seconds),curing light (Make sure the curing
light is charged), computer, monitor, x-ray I then go to the decontamination room and
switched on the autoclave (type B) , Ultrasonic bath, Illuminate magnified lamp dental water
Distiller.
I filled the autoclave and Ultrasonic bath with proper enzymatic solution after that run a test
cycle helix test (TST – control indicators strips) is placed in the mid shelve and the
temperature goes up to 134 degrees for 3 minutes (pressure 2.2 bar pressure)
After that I checked to see if autoclave works well by checking the colour of the indicator
strips from purple to green. Then I attached the strips to its specific data sheet and signed.
I went to the surgery and fill the unit bottle with purified water, and I attach it to the dental
unit. I inserted the filters,suction adaptor as I left them in the disinfectant bath during the
night. I discharged water from hand piece attachments, 3 in 1 syringe and spittoon for 2-3
minutes and also flushed the suction with 2 Litres of water.
I used alcohol wipes to disinfect all working surface and wipes like tray, dental lights, lamp
Handles, Dental chair, aspirator Unit and spittoon then I dispose of my gloves in clinical bin,
I washed and dried my hands properly then put clean gloves on.
I have covered the following areas using plastic sheets/foil to prevent cross infection on
surfaces like headrest, armrest, chair, dental Light, curing Light, X Ray machine, intra-oral
camera, computer keyboard and mouse I change these disposable covers in between every
patient.
I have prepared clear plastic tight-fitting lidded box for clean instruments from
decontamination room in to the surgery I have set up impression bath following
manufacture guidelines and lidded container for dirty instruments with enzymatic solution
finally I checked carried procedure with clinician. Following the day list which I printed out
the previous night I checked and prepared the laboratory work and ensured that the
emergency kit and all the materials are ready and have placed new 3 in 1 tip and a new
aspirator tip. I have prepared the following patient record such as dental records (chart),
medical history form, Basic Periodontal Records, signed treatment Plan, signed consent form,
Previous Radiograph.
I have prepared PPE (personal protective equipment) for myself such as disposable face
mask, disposable gloves, visors and PPE(personal protective equipment) for dentist such as
gloves, visor, mouth mask and apron then I prepared PPE for patient such as safety goggles,
single use bib and placed the mouth wash and the tissues available for the patient's use.

While I disposed of all clinical/ hazardous waste I wore clean PPE (gloves, mask, plastic
apron) the whole time.
I have collected all dirty instruments in the clinical area and placed them in the container with
a tight-fitting lid for dirty instruments.

I disposed the pouches in the non-clinical bin I disposed my gloves and washed my hands
then put on the clean glove after drying my hands, I have cleaned and disinfected the work
surface from clean to dirty and last I cleaned spittoon with 1% of bleach bases disinfectant
/alcohol wipes. I used suction unit disinfectant solution and run it until the solution in the
bucket finished and I cleaned the outer surface with the wipes and disposed all the used
wipes, paper towel and used gloves in the clinical bin then I washed my hands and put on
gloves after drying my hands then prepared the surgery for the next patient.
Decontamination of dirty instruments
I transported the contaminated instruments into decontamination room
I placed the box in the dirty zone
I put on PPE and also heavy duty gloves
I filled up washing sink with water and detergent: benzalkoniumchloride/glycerine
Water temperature around 40 Degrees and I placed the instruments in I used a plastic
nylon brush to scrub and remove debris manually and a wired brush for burs
Rinsed it under the water from detergent in separate designed rinsing sink
Placed instruments (except hand pieces) – into the ultrasonic cleaner (15 min cycle )
to remove all stubborn blood and dirt
While instruments are in the ultrasonic bath I then oil all handpieces and wipe of the
excess oil
After the cycle I used paper wipes to wipe off the instruments and checked if all of
them are free of debris under the magnifying lamp
I placed all instruments onto metal trays and placed them to autoclave (Type B
vacuum)
Set up Autoclave as follow
Temperature 134 degrees C for 3 minutes
Length of cycle 45 minute
Pressure 2.2 Bar
I then disposed of PPE(gloves, mask, visor, apron) washed my hand and put on clean
gloves
After the sterilisation is complete (I checked the printout from autoclave) then I dried
the instruments with lint free cloth and placed them in a different dry tray
After the cycle I pouched the instrument and stamp with 1 year expire date stamp and
my initials to give details who proceed sterilization in case of any issue
I placed all pouched instrument into the drawer in surgery as is a safe place to keep it
in clean zone to avoid contamination
All sterillisation process is done according to HTM 01-05 guideline

I need to work on my suctioning
To come over my weakness I need to train more with the head nurse and watch videos on
techniques.
