1. Three copies should be sent completed and submitted through district education officer to
Human Resources Office Discipline.

2. Application must be received by Human Resources Discipline not less than 4 weeks before
the date of commencement of the trip. Late application will not be processed.

3. Indemnity forms as per appendix must be completed and signed by the parents/guardians
and retained at the school.

4. A signed and stamped copy of the application form will be returned to you as
acknowledgement of receipt and as approval or not approval.

5. No trip should be undertaken until approval has been received.
