AFFIDAVIT OF ACCIDENT

I, JESSIE P. GAÑGAN, Filipino citizen, of legal age,


single, and with residence and postal address at
7148Gumamela St., Brgy. Guadalupe Viejo, Makati City,
after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I am the authorized driver of a Toyota Grandia


with Plate No. SND 2215 owned and registered under
the City Government of Makati;

2. That on December 1, 2024 at around 12:57 noon, while


responding to an emergency call from a patient
residing at Dito St., Brgy. San Antonio, Makati City,
while I was trying to back up, the front right side panel
of the said vehivle accidentally hit an electric post;

3. That as a result of the said incident, the front right


side panel of the motor vehicle suffered dent and
scratches;

4. That I am executing this affidavit to attest to the truth


of the foregoing facts and in order to inform the proper
authorities concerned about said damaged on the car
and for use in connection with car insurance claim and
for whatever legal intents and purposes this may serve.

And further affiant sayeth naught.
