AFFIDAVIT OF LOSS

I, GINA NUESTRO PANGILINAN, of legal age, Filipino, married, residing


at Kaytitinga 2, Alfonso, Cavite after being duly sworn according to law,
hereby depose and say:

1. THAT I, am a holder of a Philippine Passport ID issued by the Department of


Foreign Affairs (DFA), Manila,

2. THAT on April 2024, I lost the said Philippine Passport ID when it fell off
unnoticed from my belongings,

3. That I exerted earnest efforts looking for the said Philippine Passport ID, I
failed to find the same,

4. THAT I am executing this affidavit to inform the proper authorities and to


secure a new Philippine Passport ID in lieu of the one lost;

5. THAT I am executing this affidavit in good faith to attest to the truth of the
foregoing statements and for whatever purposes it may serve.

IN TRUTH WHEREOF, I have hereunto signed my name below, this 6 th of July


2024 at Alfonso, Cavite.
