A F FI D AV I T
I, IMELDA A. JAVIER, of legal age, married, Filipino and residing at 06
Bucandala I, Imus, Cavite, after having been duly sworn to in accordance with the law,
do hereby depose and say:
That sometime August 2007 I was confined at Philippine General Hospital and I
was required to show two (2) valid Identification Cards to avail of the discounted rate I
was applying for at that time with PCSO;
That since I only have with me my VOTER’S ID, I could not present two (2)
valid ID’s for me to avail of said benefits;
That wanting to make use of the benefits, I tried to ask around for people who
could possibly help me in securing one (1) more ID for me since I was still then in the
hospital where I was confined for three (3) months;
That the in-law of my sister-in-law, MELISSA MARTINEZ then offered to me
that she could procure a Postal ID for me in the amount of THREE HUNDRED PESOS
(P300.00) and that I could just surrender to her my Voter’s ID, 2x2 picture and my
Certificate of Live Birth which I all handed to her for purposes of securing a valid Postal
ID in my favor;
That after a week, said Melissa Martinez gave me my postal ID which I bring
with me and keep in my wallet;
That on 3 July 2008 when I was about to claim my check representing the value
of the lump sum SSS benefits due me on account of the untimely death of my daughter
THERESA A. JAVIER in Kuwait, KSA, I was required to present two (2) valid
Identification Cards and for which I gave them my Voter’s ID and Postal ID;
That on 3 July 2008, I was surprised when I was told by the staff at Imus Post
Office that the Postal ID I was presenting to them was spurious that warranted its
confiscation;
That thereafter, I tried to locate the whereabouts of Melissa Martinez to confront
her for her deception and scheme that prejudices me and I was informed that she is no
longer residing in the Philippines as she is at present in Singapore;
That I hereby declare that I have no participation nor knowledge in the processing
and production of the said fake Postal ID as it was a certain MELISSA MARTINEZ who
manipulated the production of the same in the amount of P300.00, to my great damage
and prejudice;
That I am executing this Affidavit to attest to the foregoing facts and to relate the
circumstances of how I got into possession of the fake Postal ID and to prove that I have
no participation in the production of the same and for whatever legal purpose this may
serve.
