MUFANKOLO KASSEM JEREMIE , a citizen of Démocratic Républic of Congo, and holder of
Passport No. OP1501242, valid until 24/12/2028, to come and visit the beautiful country of the
Philippines for vacation.

I am the friend of MUFANKOLO KASSEM JEREMIE.


I am taking full responsibility for my friend MUFANKOLO KASSEM JEREMIE, and she/he will
be staying in my residence here in the Philippines.
I hereby guarantee and assume that my friend will not become a public burden during her/his
