MEDICAL REPORT
Re: Nwakanma Chisom Blessing / Female / 20 years
The patient, Nwakanma Chisom Blessing, has been under our medical care for the management
of chronic chest pain and asthma. Her condition has been closely monitored, and it is critical to
note the following observations and recommendations:
Medical History:
The patient has a long-standing history of asthma, with recurrent exacerbations requiring
medical intervention. She has been experiencing chronic chest pain, which necessitates regular
monitoring and proximity to advanced medical care.
Current Status:
Recently, there has been an increase in the frequency and severity of her asthmatic attacks,
further complicating her condition and increasing the risk of respiratory emergencies.
Recommendation:
In view of the criticality of her condition, it is strongly advised that she remains close to a well-
equipped medical facility that can provide immediate and appropriate care in the event of an
emergency. Moving to a distant location, may pose a significant health risk due to the
unavailability of her regular medical team and the potential delay in accessing emergency care.
For her safety and well-being, it is recommended that she be redeployed to a location closer to
her residence in Lagos, where her healthcare needs can be promptly addressed.
