 Keep these four documents handy for Medical:

1 ) Copy of the passport of child ( front and back page )

2 ) Copy of the vaccination waiver form

3) Print of the embassy appointment e- mail confirmation .

4 ) Medical examination report (MER) of child received from orphanage.

4 )Passport of child ( front and back page )

5) Adoption order

6) Conformity certificate

7 )NOC

8 )FORM C from Hotel where you are staying in Delhi

9) Return Ticket

10) Email on which you want the exit permit

*You will not need Frro exit permit if you hold an Indian passport.

*It gives a window of 15 days to exit the country.

*Frro may take 3-4 working days to process

 Embassy documents:

1.Electronic Immigrant Visa Application (DS-260) confirmation page: The DS-


260 must be updated before the visa interview to include the child’s passport
details and other changes, if any.

2.Child’s birth certificate

3. Updated Fingerprint approval


4.The Original court order and a copy of the court order

5 2 recent photographs of the child (photograph should be clear, measure 2


inches square with the head centered in the frame and must have only a
plain white background)

6.NOC from CARA

7. Article 23 letter from CARA.

8. Police Clearance Certificate (PCC) issued by Regional Passport Office, only


if the adoptive child is/or will be over 16 years of age on the day of interview.

9.Visa Fee of $325 (in cash – Indian rupees or U.S. dollar or by U.S. credit
card)

10.Medical examination report of the child
