son/daughter ___________________________ in the District Festival of Talents for
English and Filipino on February 26, 2025 at Caraga Central Elementary School,
Poblacion, Caraga, Davao Oriental.
I have considered the benefits that my son will derive from her participation
in this activity provided that due care and precaution will be observed to ensure the
comfort and safety of my daughter and that DepEd employees and personnel may
not be held responsible for any untoward incident that may happen beyond their
control.
