This certificate does not entitle a foreigner to perform work in the territory of the Republic of Poland, except in situations referred
to in Article 5dp paragraph 1 of the Act of 20 April 2004 on the promotion of employment and labor market institutions Entity

address of the foreigner's accommodation during his stay in the territory of the Republic of Poland and submit a copy of the
foreigner's travel document, together with the visa on the basis of which the foreigner entered the territory of the Republic of
Poland and the documents containing the date of crossing the border of the Schengen area.
If the notification is made correctly, the foreigner is obliged to perform work under the conditions specified in the certificate and
to wait for a decision on the issuance of a work permit for a season, from the date of completion of the required documents to the
date of delivery of the decision. Pursuant to Article 50h(1)(3) in connection with Article 80p(1) of the Act of 30 April 2004 on
promotion of employment and labor market institutions, the entity entrusting work to a foreigner and the foreigner are obliged to
conclude a written contract on terms and conditions consistent with those indicated in this notice before the foreigner starts
work.
Work under the conditions indicated in the above notification may be considered logana only if the foreigner has a Valid way
issued for the purpose referred to in Article 50 (1)(Sa) of the Law on Foreigners of December 12, 2013, or if the foreigner is legally
arriving on the territory of the Republic of Poland in the framework of visa-free movement.
This certificate does not entitle you to cross the border of the Republic of Poland.
If an application for a permit to work in the territory of the Republic of Poland has been entered in the register of applications, the
proceedings in this case shall be discontinued by operation of law (void 10pkt 1-3), if the entity entrusting the performed work
does not present a copy of a valid document authorizing the cudestemic to stay in the territory of the Republic of Poland.
