AFFIDAVIT OF LOSS
I, LLLLL of legal age, Filipino, married, and a resident of TTTT,
after having been duly sworn to in accordance with law, do hereby
depose and state that:

1. I am a holder of a Philippine National ID;

2. Sometime last month, in January, I noticed that the


abovementioned ID was not in my possession;

3. Despite diligent search and efforts to locate the said ID, I could
not find it such that I now believe that it is lost beyond recovery;

4. I am executing this Affidavit to attest to the truth of the


foregoing and to support my request to have a new Philippine
National ID issued in my name, and for whatever legal purposes
this may serve best.
