POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS THAT I, <name of the accountholder>, S/o D/o
W/o Mr. <name> at present residing at <complete address> DO HEREBY APPOINT AND
CONSTITUTE Mr./Mrs./Ms. <name of the attorney> S/o D/o W/o Mr. <name> at present
residing at <complete address> (hereinafter called the ‘Attorney’ who has subscribed
his/her signature hereunder in token of identification) as my true and lawful Attorney in
my name and on my behalf to do any or all of the following acts, deeds and things namely:

1. To open Savings Bank/Current Account/Fixed Deposit/Recurring Deposit Account in


my name with <name of branch> branch of Ujjivan Small Finance Bank Ltd.

2. To operate on the Savings Bank/Current Account/Fixed Deposit/Recurring Deposit


Account so opened in my name with <name of branch> branch of Ujjivan Small Finance
Bank Ltd. (hereinafter called the ‘said account’);

3. To draw cheques on the said account;

4. To deposit and/or discount cheques and other instruments in the said account and for
this purpose to endorse on my behalf cheques, drafts, pay orders and other
instruments payable to me, to make deposits from balances available in the account in
my name and to renew such deposits for such periods as may be given in writing by the
mandate holder;

5. To certify balance confirmation statement and statement of accounts issued by the


Bank in respect of the said account;

6. To give instructions in writing involving debits to the said account, transfers there from
etc.;

7. To enter into, make, sign, execute, deliver, acknowledge and perform any agreement,
deed, writing or thing that may in the opinion of the Attorney be necessary, proper and
expedient for the aforesaid purposes; and

8. To do all such acts, deeds and things as may be necessary for or incidental in respect of
the aforesaid.

9. The Attorney shall, however, while acting under this Power of Attorney, ensure that
the Attorney acts in accordance with the Instructions and directions that may be issued
from time to time by Reserve Bank of India or the Government of India or the Bank or
any other body.

I hereby undertake to ratify and confirm all and any acts that the Attorney does or causes
to do on the basis of this Power of Attorney.
