Croydon Health Services provides integrated NHS services to care for people at home, in schools, and
health clinics across the borough as well as at Croydon University Hospital and Purley War Memorial
Hospital.
CUH provides more than 100 specialist services and is home to the borough’s only Emergency Department
and 24/7 maternity services, including a labour ward, midwifery-led birth centre and the Crocus home
birthing team.
Purley War Memorial Hospital (PWMH) in the south of the borough offers outpatient care, including
diagnostic services, physiotherapy and ophthalmology services run by Moorfields Eye Hospital, alongside
an onsite GP surgery.
Our experienced district nursing teams, Allied Health Professionals and community matrons look after for
people of all ages across Croydon and our Children’s Hospital at Home cares for children with long-term
conditions without them having to come to hospital.

DEPARTMENT DESCRIPTION
1. INTRODUCTION
The post has arisen due a vacancy in Community Paediatrics. The appointee to the post of Consultant
Community Neurodisability Lead Paediatrician will add to a current team of five Consultant Community
Paediatricians. The service is integrated with acute Paediatrics which has seventeen Consultants.
Together, both teams take on the co-ordination and delivery of a high quality Paediatric Service in
Croydon.
2. SPECIFIC AREAS
The post holder will be employed by Croydon Health Services and will work in a multi-agency team within
Croydon.
The appointee will work closely with special school nursing, therapies, social services, public health and
voluntary agencies to improve the health and outcomes for children with complex health needs in
Croydon. The post holder will be expected to work with hospital colleagues and develop links across South
West London.
The post is full time but flexible or part-time working requests will be considered.
The post holder is expected to contribute and support the CMS (Community Medical Service) staff and
participate in multidisciplinary and multi-agency work, including children safeguarding duties in order to
meet the needs of Croydon children and young people. The post holder is also expected to contribute to
Continuing Care Panel.
The post holder is based at Lennard Road, the community services base and St. Giles School. They would
be provided with IT support, as well as the new Patient Pathway coordinator, our revised secretarial
support, utilizing pooled typing resources and pathway coordinators.
3. CLINICAL ORGANISATION
Croydon is a large London borough with a resident population of approximately 390,500. Croydon has a
relatively young population with about 26% aged less than 19 years, a slightly higher proportion than the
national average. Croydon is ethnically diverse. In the 2006 Census, 40% of the population comprised black
and minority ethnic groups. Overall the population enjoys better health than ever before.
The annual report of the Director of Public Health Medicine in Croydon 2011 identified some areas of
special concern and health care priorities. In 2014 approximately 4500 babies were born to women
resident in the borough of Croydon. Croydon has a high risk maternal population with a high fertility rate
and a high proportion of babies born with low birth weight.

Croydon Local Authority had at the end of October 2023 506 Children Looked After. The rate of Children
Looked After (CLA) per 100,000 under 18 population was 56.1. In addition, there are a number of children
who are looked after by other Boroughs but are resident in Croydon.
There are established policies and guideline for staff dealing with LAC and safeguarding issues. There is a
good multi-agency support for LAC via Local Authority virtual schools team, and integrated working with
the immunisation and specialist nursing teams for safeguarding and looked after children.
Croydon has about 2,220 children with Education Health and Care Plans (EHCP).
The Community Paediatric doctors provide the medical support and advice to local schools and children
centres. Access to hospital and inpatient facilities is available at Croydon University Hospital for
investigations and acute care.
There are many infant, junior, secondary and independent schools in Croydon. There are no routine
mainstream school entry medicals however; outreach support is available at mainstream and special
schools for the Team around the Child Meetings. Selective school medicals are undertaken by the medical
staff at special schools.
There is also a wide range of special schools and units to provide facilities for children with special
educational needs:
Red Gates School & For children and young people with severe learning difficulties
Priory School and Priory

St Giles School For children with profound and complex and physical
disabilities
Beckmead School For children with emotional and behavioral problems
Rutherford School A special school run by SCOPE for children with multiple
disabilities from age 4-19 years.
Addington Valley Academy Addington Valley Academy is a co-educational special school specialising
in autism and complex needs and addresses the social communication, cognitive, sensory and social,
emotional, and behavioural needs of pupils aged 2 – 19.
o Units for children with autism at St Nicholas school and Castle Hill School.
o A Unit for children with visual impairment at Oval school.
o A Unit for children with hearing impairment at Kingsley school.
o Language Unit at Applegarth School

o Communication Unit at Winterbourne and several SEN new units
Other special units and provisions for children with behavior social communication and learning needs
are children centres, nurseries and for Key stage 1 and 2.
Croydon has expanding schools at mainstream and special provisions, to meet the changing population
needs. There is input for children with physical disabilities and medical needs at various schools in the
borough.
Services for children with special needs are provided by the local authority day care centres. The local
authority has a policy of positive integration into mainstream school. There are active portage and early
intervention teams who support the families of children with special needs. There is a continuing care
panel and support services for physical/medical needs.
The Early Years team works closely with Community Paediatricians to ensure that the Children Act
principles are upheld. There is also the early notification of children who are likely to have special
educational via the Children’s Coordinating Team (CCT).
There are a number of children’s centres and nurseries, some of which offer specialist input for children
with physical disabilities and medical needs.
The referrals are received from various services including GPs, therapists, schools and hospital
Paediatricians. Specialist clinics in social communication disorders, motor disorders, neurodisability,
epilepsy, enuresis and behavioral concerns are run. There are second level referral clinics in the
community staffed by the community speciality doctors and paediatric trainees for initial assessment,
planning and monitoring of children who may have special needs and developmental difficulties.
Additional services
Croydon Health Service employs a nurse specialist for children with special needs and there is a Children’s’
Hospital At Home team. There is also a well-staffed specialist school nursing team at St Giles School and
at the Croydon Opportunity play group.
There is a Centre for Children’s Services (Malling Close in the community near Woodside). A multi-
disciplinary team work from this centre, carrying out developmental assessment of children with
developmental problems.
Laboratory and X-Ray facilities are available at Croydon University Hospital (CUH) for those children who
need them.
The children’s safeguarding in the Croydon Health Service is Consultant led. Children with suspected non-
accidental injuries or who are vulnerable are seen for assessment/medical examination in the community
by the medical team. Referrals are taken from Social services.
There is a purpose built facility, the Saffron Suite on the CUH site, for medical examinations of children
with suspected physical abuse.

Liaison between Services
There are close working arrangements with colleagues in CUH and members of the multidisciplinary team,
health visiting, school nursing, Speech and Language Therapy, Occupational Therapy, and Physiotherapy.
Multi-agency working with CAMHS, Local Education Authority, Social Services Public Health and other
frontline children’s services is actively promoted.
The Child Health Team support the patient records and archiving system. They also enter all the Croydon
New Births, Immunisation, Newborn Screening results electronically and they work closely with
community Paediatricians to maintain the electronic Special Needs Register.
All consultants are expected to be committed to teaching and to play their part in the delivery of the
paediatric trainee RCPCH curriculum. These activities would include the planning and review of teaching,
and the assessment of Paediatric Trainees via RCPCP Portfolio. This includes out-patient clinic
observations.
The need for flexibility is acknowledged by the Trust and, for example, within a service department it is
possible that the overall teaching commitment to be unevenly allocated between staff to achieve optimal
teaching and service quality.
The appointed physician will participate in teaching post-graduate doctors and other groups at Croydon
Health Services. In the community paediatric service this can involve teaching to Health visiting, specialist
nurses and therapists.

5. RESEARCH AND DEVELOPMENT
Involvement in high quality research & development is strongly encouraged at Croydon Health Services
NHS Trust, as this supports both the delivery of high quality clinical care.
An important aspect of NHS care is research into human subjects to help develop innovative approaches
that would benefit both the patients as well as the wider NHS agenda. Towards this end, there is an
expectation that clinicians would be willing to take on clinical research, supported by the R&D
department. We would expect all divisions to have a balanced portfolio of research activity, including
CLRN studies, commercial studies and health service research. MDT involvement in research is strongly
encouraged.

The postholder will be expected to contribute and participate in the Department’s ongoing clinical audit
programme, and will carry out all necessary administrative duties associated with the care of their
patients. The postholder holder also has a general duty of care for the health, safety and well-being of
work colleagues, visitors and patients within the hospital, in addition to any specific risk management or
clinical governance accountabilities associated with this post.
The postholder will be expected to comply with the CME requirements of the relevant Royal College.
Study leave may be requested in accordance with the Trust’s Policy for Continuing Medical Education.
Annual leave may be taken in accordance with the Trust’s Guidelines on Leave Arrangements for Medical
and Dental Staff. To ensure the smooth running of the service the postholder will ensure that, in
conjunction with colleagues, adequate arrangements are made to cover planned absences.
The post is covered by the current Terms and Conditions - Consultants (England) 2003, the Terms and
Conditions of Service of Hospital Medical and Dental Staff (England and Wales) and Trust Policies and
Procedures where relevant. Appointment is conditional upon a satisfactory medical assessment, which
may include an examination along with pre-employment clearances in line with NHS Employment
Standards Checks.
Appraisal is an important aspect of your development as a doctor and forms a key part of your

10. MENTORSHIP
CUH offers newly appointed Consultants 4 free coaching/mentoring sessions. This is in line with the GMC’s
recommendations for Good Medical Practice. The success of coaching and mentoring is aimed at
establishing effective relationships with mentors based upon mutual respect, honesty and understanding,
combined with structured questioning to enable the mentee to find their own solutions.

JOB DESCRIPTION AND OUTLINE JOB PLAN
GENERAL
This is a Replacement post. The remit of the postholder will be to work within the multi–disciplinary team
providing and further developing community medical service. In addition, the individual will be required
to play an active role in the general workload of the department while having a significant commitment
to teaching and training of junior staff.
PURPOSE OF JOB
The post holder will be based at Lennard Road, Croydon Health Services and other work locations
including off site working may be agreed in your Job Plan where appropriate. The proposed Job Plan is
attached. There will be an average of 8.5 Programmed Activities (PA) of Direct Clinical Care (DCC), with
the remainder 1.5 PAs being allocated for Supporting Professional (SPA) and/or other Activities.
Although this is a Community Paediatrics post, we also welcome applications from General/ Neurology
Paediatricians willing to provide leadership in the care of children with complex needs. Some outpatient
clinics can be modified to accommodate general and speciality interests, and consideration can be given
to acute paediatric work including covering the paediatric emergency department if desired.
Please note that job plans are subject to review by your Clinical Head of Service and Clinical Director.
As a senior employee of the Trust you will work in close co-operation with, and support other clinical,
medical, professional and managerial colleagues in providing high quality healthcare to the Trust’s
patients. Integral to these responsibilities are the following:-

1. To provide community medical service and this includes all aspects of treatment and relevant
management duties for the proper functioning of the department.
2. Responsible for covering colleagues’ periods of annual leave and short-term sickness.
3. Clinical supervision of junior medical staff as a shared responsibility with other consultant colleagues.
4. Responsible for carrying out teaching, examination and accreditation duties as required and
contributing to Continuing Medical Education and clinical governance initiatives.
5. Responsible for out-patient clinic commitments in peripheral hospitals as agreed by Croydon Health

6. To contribute to departmental research interests in accordance with the Trust’s R&D framework.
7. To take an active role in the formulation, implementation and monitoring of the Trusts Objectives. This
may include assisting with discussions regarding service plans and developments with both internal
and external agencies.
8. To conduct all activities within the contracted level of service and operating plan for the Department.
9. To exercise professional leadership for all staff working in the specialty. This will include fostering and
developing projects focused towards these ends and ensuring that the momentum of post
qualification professional education and other appropriate training is maintained.
10. To work in conjunction with clinical and professional colleagues to ensure that the productivity of staff
within the specialty is maintained and their job satisfaction is enhanced. This may include involvement
in the appraisal process for junior colleagues.
11. To undertake management duties as agreed with the Clinical Director and Head of Service.
12. To be aware of, and comply with, all Trust infection prevention and control policies, to include hand
hygiene, personal hygiene, environmental and food hygiene. To undertake annual training/updates
in infection prevention and control.

Clinical Duties

(a) This post holder will primarily work with children in the assessment and management of children
with neurodevelopmental disorders and complex neurodisability and will be the named doctor for
St. Giles School for children with moderate to profound learning disabilities and complex health
needs. The post holder will also Lead in the Neonatal high risk children follow up clinics jointly with
Neonatal consultant, Croydon University Hospital and contribute to National Audit.
(b) Development of care pathways for conditions such as cerebral palsy and transition medicals for
children with complex needs.
(c) Supervision and training of staff and trainees in child developmental assessment / care planning for
children with SEN/ to continue the newly establish St Giles feeding clinic for children with GOR
and complex feeding problems.
(d) Medical assessment of children who have been referred for safeguarding reasons and child
protection advice from social services.
(c) Should be able to provide medical examination and written reports in connection with statutory
assessment of special educational needs (EHCP)
(d) The appointee will participate in the medical assessment and management of children with
neurodevelopmental problems / Autism Spectrum Disorder
(e) There will be opportunities for developing special interests in a chosen clinical area.
