you were found neglecting the patient and due to your negligence patient found
dead when doctor went for follow up.

3. Patient Urmila urine culture sample sent but not entered in lab register.
Your seniors tried to aware you of the negligence through verbal warnings at various times,
but this is getting intolerable for me now since your fellow employees and team members

Due to continuous negligence of responsibilities, I am issuing you this first formal warning
