I, RAMON A. RAZAL, of legal age, married, Filipino, and a resident of 18 San Luis
Ave., Jubileeville, Brgy. Masaya, Bay, Laguna 4033, after having been duly sworn to
in accordance with the law, hereby depose and say:

1. That I no longer have a source of income from employment after my


retirement from government service on August 31, 2021;
2. That as a consequence, I have not filed my Tax Return for the past four years;
3. That I am executing this affidavit, to attest to the truthfulness of the
foregoing facts and for all legal intents and purposes.
