AFFIDAVIT

____________________________________ after being duly sworn to in accordance with law do hereby depose and
say:

1. That I am a 4th year student of Bachelor of Science of Office Administration of Bacolod City College:
2. That of the requirement for graduation is to undergo an On-The-Job Training (OJT) program for a minimum but
not limited 600 hours:
3. That, I promised to render the necessary work required of the OJT program to the best of my ability:
4. That while performing the OJT, and for the duration thereof, I shall abide by the rules, obey orders and
instructions incidental and necessary thereto:
5. That I shall be punctual in my attendance and shall abide by the evaluation given by the Host Training
Establishment (HTE) based on such OJT:
6. I understand and declare unconditionally that there exist no employee/employer relationship between the Host
Training Establishment and my position as trainee/apprentice and so hereby hold the HTE, its officers and
personnel free and harmless from any cause or causes, claims or otherwise, arising from or in connection with
my OJT, which I might have, including money claims, it being understood that my OJT with the HTE serve only as
an additional job exposure to the world of work.
7. I further hold my position as ON-THE-JOB trainee free and harmless from any liability whatsoever during the
period of my training or immediately thereafter while on my way to and from the office.
8. While I understand that the HTE has no obligation, legal or otherwise, to absorb me in its workforce, this does
not however prejudiced the possibility of my becoming its employee if their should be vacancy which finds me fit
and qualified.
9. I understand that all records and documents of the HTE and all information pertaining to its business or affairs
are strictly confidential and I promise that I shall not make any unauthorized disclosure or reproduction of the
same at any time during or after my On-The-Job training with the Host Training Establishment.
