We offer a number of specialist services and play a key partnership role with Greater Manchester, Stockport and East
Cheshire. With an annual budget of around £300 million we provide healthcare for residents in Stockport, East
Cheshire and North Derbyshire as well as patients we treat from other boroughs in Greater Manchester who choose
our services. We are a specialist `hub` centre for emergency and high risk general surgery, one of only four in Greater
Manchester and covering the South East sector of our region. Our stroke services have been officially ranked as the

Introduction:

We are a proactive and passionate acute medicine team of 15 consultants. We are looking to expand our
workforce to ensure flexibility to deal with our front end pressures, while at the same time maintaining a
favourable work life balance. The team is not only keen to welcome acute medicine CCT doctors, we are also
open to specialist colleagues who have other accreditation including diabetes, intensive care medicine,
cardiology, gastroenterology or rheumatology who want to be part of the acute medicine team as a hybrid post.
We are keen to create flexible job plans for talented and enthusiastic candidates.

We have a multidisciplinary ethos and are proud of the excellent working relationship with our Emergency
Department and other specialties. A key objective on which we are making progress with Stockport Together is
breaking down the hospital barriers and forming even stronger partnerships with GP and community services to
understand and use our wider healthcare network to its optimal level. We share a business group with
Community Services to aid this process.

We are a multi-professional team, very active in both undergraduate and postgraduate teaching, and actively
training other allied health professionals to develop more clinical roles such as pharmacist consultants as well as
expanding our team with more recent and evolving professionals such as Physician Associates.

We were chosen in July 2015 as one of four ‘specialist’ hospital sites in Manchester, following a unanimous
decision by the 12 Greater Manchester clinical commissioning groups (CCGs) as part of the Healthier Together
programme.

A programme of reform of unscheduled care in the Trust and across Greater Manchester is underway which
includes investment in and expansion of the medical specialties to complement the recent rapid expansion of
Acute Medicine; this will facilitate more robust in-reach to the AMU streamlining the care of patients to the
appropriate specialist on admission.

Job Plan:
In the first year of appointment new consultants are as part of Trust Policy on a 10 PA job plan, given 1.5 SPA, as
core SPA (i.e. for revalidation and basic educational work). The exact job plan will be based on discussion with
the successful applicant.
If any specific additional SPA activity, in line with trust needs, is identified at initial job planning this will attract
additional SPA time e.g. Clinical governance & mortality review SPA etc.
Upon completion of the first consultant year the appointee will have a review of SPA activity through a job
planning process and dependent on SPA activity additional SPA allocation may be granted.
All Consultants are required to work flexibly in undertaking their duties. This will require working during the
evenings and weekends to ensure effective delivery of service and its sustainability. The final job plan will be
discussed with the appointed consultant by the Clinical Director.

The acute medicine consultant team is supported with the presence of a resident doctor, for each consultant, on
ward rounds on the acute medical units.
Additionally, the acute on call team, mainly from the general medical specialties, works alongside the Acute
Medicine Team, on the assessment and treatment of acute admissions during the day (9am-9pm). The acute
medicine resident team is not on the on-call rota but provides daily additional cover on AMU and SDEC 7 days
per week.

JOB DESCRIPTION

Main Duties and Responsibilities:

The main duties of this post are the assessment, early decision-making and close co-ordination of the clinical care of
patients in the acute medicine bed base, and the provision of leadership to the acute medical take. To achieve this
the team work flexibly within the acute medical bed base: Acute Medical Unit (AMU), Acute Frailty Unit (AFU,
alongside the frailty team), Same Day Emergency Care (SDEC) and the Emergency Department (ED).

Clinical duties are performed according to an agreed job plan which describes weekly working in a rotational
arrangement with other acute physicians in terms of DCC and SPA activity. The job plan makes explicit allowance for
clinical administration within the working week in the form of 1 PA based on a 10PA job plan or above. The rota for
the team is done within the team to allow for cross cover and continuity of care. This post involves premium working
hours including evening and weekend working. The post is offered on a full-time basis. Office accommodation and
medical secretarial/clerical service will be provided.

ACUTE MEDICINE CLINICAL AREAS OF WORK

Acute Medical Unit:

Our Acute Medical Unit is a new, purpose built 46 bedded ward area located next to the Same Day Emergency Care,
Emergency department and ITU areas.
Our current rota means we work 09:00 – 17:00 most days with one late shift approximately every 2 weeks finishing
at 21:30. There are 3 consultants doing a ward round each morning with Resident medical/ANP support. The
Physician of the Day (POD) shift is from 07:00 – 15:00 and 14:00 – 21:30 and provides medical advice in the
Emergency Department as well as leadership to the acute medical take. We work flexibly across all areas depending
on demand. Currently we do not cover overnight on call (provided by general medical on call rota).

Weekend Cover: The Acute team and general medical on call team are two separate rotas which work in parallel. The
Acute rota is currently a 1:6 frequency. 2 Acute Medical consultants work from 08:00 to 17:00 on AMU on Saturday
and Sunday, during which time we are on call for issues arising within the ED department or on the other acute
medical areas until 17:00.

The general medicine on call Consultant supports the acute medicine consultant from 08:00 to 12:00 and then takes
over the on call from 16:00. There is also a cardiology consultant ward round in the mornings involving a CCU ward
round and in-reach into AMU. We also have a 24/7 gastroenterology on-call rota for upper gastrointestinal bleeding
requiring emergency endoscopy.

This arrangement including frequency and degree of out of hours cover can change dependent on the demands of
the service.

Same Day Emergency Care Unit:

SDEC is open 7 days per week and sees between 40-55 patients per day. It is currently located at the top of the AMU
whilst our new Emergency and Urgent Care Campus (EUCC) is being constructed. It is staffed from 09:00 to midnight

(22:00 at weekends) with registrar and nursing presence. Consultant presence is till 17:00 (and occasionally 19:00) on
weekdays. SDEC will manage patients with a variety of acute conditions and will also review patients discharged from
AMU who need follow up within a few days. GP referrals are triaged by senior sisters, registrars or consultants and a
decision is made as to whether SDEC is the right choice of care pathway.
In addition to the consultant on SDEC, an acute physician takes all GP phone calls and referral bleeps from 13:00 –
17:00 on weekdays. This provides a single point of call for medical referrals as well as advice and guidance,
signposting patients to the most appropriate service.

Emergency Department:

We have fully embedded Acute medical consultant in-reach into the ED. The POD role provides support for early
clinical decision making, the management of acute medical emergencies, and the identification of patients suitable
for early discharge via SDEC, or from the ED itself. It also provides leadership for the take and supports the flow of
patients out of ED and through the acute care pathway. The number of patients attending ED per month is circa
8300.
The ED has 16 majors cubicles and a 4 cubicle resuscitation area. As part of the Healthier Together program, Stepping
Hill is the main surgical centre for non-elective and elective surgery in the South east sector in Greater Manchester.
We have approved plans to increase the size of our ED resus footprint and to re-design the front of ED to achieve the
aims of the Urgent treatment Centre specifications laid out in the Greater Manchester reorganisation of services.

Acute Frailty Unit:

The Acute Frailty Unit (AFU) is a 16 bedded ward area located adjacent to the AMU. The AFU admits patients who
are aged 65 years or older and who have a Clinical Frailty Score of 5 or more, directly from the Emergency
Department. The aim of the AFU is to provide rapid and comprehensive MDT input for frail elderly patients.
Disciplines involved include physiotherapy, occupational therapy, social workers, discharge coordinators, pharmacy
as well as nursing and medical input. The patients are selected and clerked by the Frailty Intervention Team which is
made up from experienced Advanced Clinical Practitioners. The acute physicians perform daily ward rounds and
review acute frailty admissions on Mondays to Fridays, supported by a senior clinical fellows, junior clinical fellows
and physician associates. Outside normal working hours new admissions receive their senior review in the same way
as other acute medical admissions, by the POD or general medical consultant on call. There are plans to obtain
geriatrician input into the unit.

NON CLINICAL COMMITMENTS

Medical Education :

The unit consistently gets excellent feedback from medical students and resident doctors for our teaching provision.
The unit has third year and fifth year students attached to it for most of the year. The consultants organise weekly
dedicated teaching sessions for the students which the appointee would be expected to participate in. In addition,
we organise a weekly acute medicine meeting which is well attended, and have a consultant led monthly rota for
morbidity and mortality reviews.

The post-holder will participate in audit and medical specialty meetings and undertake appropriate administrative
duties. You will be expected to support informal and formal staff teaching and learning across the multidisciplinary
team.

BACKGROUND INFORMATION FOR STOCKPORT NHS FOUNDATION TRUST

GEOGRAPHICAL BACKGROUND:

Stockport constitutes one of ten boroughs in Greater Manchester and its proximity to the Manchester Medical
School has enabled the development of close working relationships with various University Departments.

The general environment in Stockport and the surrounding countryside is a pleasant one, with the Cheshire Plain to
the South West and the High Peak of Derbyshire to the South East and many stately homes and parks. There are
numerous desirable residential areas and excellent facilities for all types of education. There are superb transport
links including Manchester airport a short distance away.

THE TRUST:

Stockport NHS Foundation Trust was established on 1st April 2004. We have over 17,000 Members. The Trust
provides hospital and outpatient care in acute and community care for children and adults across Stockport, the High
Peak area of Derbyshire and community services to the population of Tameside and Glossop. The catchment
population of our hospital services is about 350,000. The catchment of our community services is about 550,000.

Since becoming a Foundation Trust in 2004 we have used our financial freedoms to improve facilities for patients.
The Trust has built a £25million Cardiology and Surgical Unit and the £10million first phase of our Southern Sector
Development. We have also built a new Paediatric Emergency Department and completed a new extended Neonatal
Unit. The Trust has also invested in additional car parking facilities at a cost of £2million. We are currently building a
purpose built Urgent Care Centre due for completion in 2016.

The Trust employs nearly 5,800 staff. Our major hospital is Stepping Hill Hospital located on the A6 south of
Stockport town centre. The Trust also provides hospital services at the Devonshire Centre and The Meadows in
Stockport, the Corbar Maternity Unit based 18 miles away in Buxton and The Lakes and Shire Hill Hospital in
Tameside and Glossop.

Specialist Hospital Status:

We were chosen in July 2015 as one of four ‘specialist’ hospital sites in Manchester, following a unanimous decision

Being a ‘specialist’ hospital will enhance our general surgery, anaesthetics, critical care and emergency medicine for
the benefit of people in Stockport, High Peak, Cheshire and across Greater Manchester.

We are a designated A&E trauma unit and have a fully equipped critical care unit. Our specialist hyper acute stroke
centre is one of only three in Greater Manchester.

Accreditation:

Prospective candidates for this position should possess full MRCP or its equivalent and have demonstrable
experience in Acute Medicine. You should be on the Specialist Register of the General Medical Council,
with a CCT in Acute Internal Medicine (AIM) or General Internal Medicine (GIM) or about to obtain this
within six months of the date of interview. We welcome applicants with GIM accreditation and a specialist
interest.
It is expected that the newly appointed consultant will complement the current clinical and academic
interest of the department.

Continuing professional development:


The Trust supports the requirements for Continuing Medical Education as laid down by the Royal College of
Physicians and is committed to providing time and financial support for these activities. The appointee is
expected to participate in CPD and fulfil college requirements.

The Trust is committed to ensuring that newly appointed consultant staff are given appropriate support
within their role. This includes opportunities for the post holder to access appropriate clinical/peer
mentoring and to fulfil ongoing career development and qualifications. The post holder should discuss
opportunities for mentoring with their Clinical Director; this will ensure that the support available best
meets the post holder’s individual needs.

Research, Development and Audit


There is an active clinical audit department in the hospital with quarterly inter-departmental meetings at
which there is regular medical input. The department is also involved in its own internal audit and more
extensive inter-departmental audit is a regular ongoing exercise.

In response to recent national and regional changes of the NHS Research and Development funding,
Stockport NHS Trust is currently developing an integrated R & D information and support infrastructure.

It should be noted that a Research and Development fund exists within the Trust and that a Research and
Development Coordinator and Committee, chaired by a Consultant Epidemiologist exists to help this with a
research interest.

The successful candidate ideally should have an interest in research and be prepared to play a role in
developing and contributing to R & D initiatives within the department.

A willingness to take lead in implementing the findings of R & D in clinical practice is welcome and the
successful candidate will be expected to actively contribute to debate within the Department on methods
for enhancing the effectiveness of the services delivered.

Divisions

Integrated Care Division: Community services.

The Integrated Care Business Group provides a range of hospital and community medical services

The Business group manages the full range of nursing community services in Stockport and works closely with
primary care & social care.

Medicine and Urgent Care Division

The medical specialties include regional hyper acute stroke services, Cardiology, Chest Medicine, Gastroenterology,
Diabetes and Endocrinology, Clinical Haematology, Rheumatology, Nephrology, Neurology, Rehabilitation, acute
and emergency medicine through the Emergency Department, Acute Medical Unit, Medical SDEC and the acute
frailty unit.

The majority of hospital services are delivered from the Stepping Hill site, with some inpatient beds on the Cherry
Tree site and outreach to Buxton Hospital is also provided.

Surgical, Critical Care and Gastroenterology Division

The surgical specialties include Urology, General Surgery, ENT and Trauma and Orthopaedics. The trust is a healthier
together provider site and also a trauma unit.

As part of providing care of emergency general surgical patients, the trust runs a consultant of the week model for
better continuity of care. There is a dedicated Surgical Assessment Unit, which actively pulls patients from A&E so
that it can improve flow for emergency patients. An emergency / CEPOD theatre and a trauma theatre is available for

emergency work every day allowing increasing consultant input into emergency work thus reducing out of hours
emergency workload particularly after midnight. We are in the process of setting up pathways to ensure the
fundamentals of Heathier Together can be delivered.

There are also additional subspecialty lists for special interest cases to treat fracture necks of femurs, foot and ankle,
shoulders and elbows and hand surgery. The trust features highly in reduction of mortality of fracture neck of femur
patients - among the lowest in the country. This is with collaborative care provided by the Orthopaedic and
Orthogeriatricians. There is a dedicated Fracture Neck of femur ward and Elective Orthopaedic Unit. The hospital
also provides acute and elective spinal care, who works closely with the spinal team at Salford Royal FT spinal unit.

Our Critical Care Unit consists of 6 ICU beds and 8 HDU beds. These staffed beds are used flexibly to accommodate
level 2 & level 3 patients as required.

Within the business group also falls the department of anaesthesia and critical care. The trust has a purpose built
anaesthetic pre-operative assessment service run by nurse practitioners. There is a well-established Acute and
Chronic Pain service with 3 chronic pain consultant’s, two acute pain specialist nurses and two chronic pain specialist
nurses; the service has a lead consultant.

We have a JAG accredited endoscopy unit providing all forms of endoscopy upper and lower along with
bronchoscopy. We also perform capsule endoscopy.
The Trust has implemented a GI bleed rota for the weekends to manage Acute GI bleeds by the appropriate specialist
teams. With further expansion of gastroenterologists we will implement a 7 day service. They also treat a wide range
of hepatic and gastroenterological conditions.

The department of Urology has been providing urological cancer care, genito-urinary, incontinence care, robotic
surgery for the population of Tameside and Glossop, Stockport and East Cheshire.

Women and Children’s Division

Maternity comprises a comprehensive range of maternity services to the women of Stockport and the High Peak area
of North Derbyshire and Gynaecological Surgery. The service works in partnership with General Practitioners and the
Health Authority and there is a small base in Buxton providing Maternity services. Gynaecology comprises an
infertility service; inpatients; outpatients; direct access hysteroscopy; colposcopy; and access to Ward D5 and Short
Stay Surgical Unit.

Children's services - we have a purpose built Children's unit housing 3 acute wards including medical and surgical
specialties, plus an assessment and observation unit and a 2 bedded HDU. On the ground floor is an outpatient unit
with a day case investigation suite and integrated CAMHS and therapy services. Stockport has a unique paediatric
model where community and acute services are integrated so that a child only has one paediatrician, and we have an
in house acute children's community nursing team. We also have extensive community services including a child
development unit, a children's learning disability team and a respite provision off site for children with severe
physical and learning disabilities.

Neonatal unit - our unit was fully refurbished and extended in 2009 to provide a state of the art facility with 16 cots
providing intensive, high dependency and special care as part of the North West Regional neonatal cot network.

Clinical and Support Services Division:

The Diagnostic and Clinical Support Services provide Pharmacy, Physiotherapy, Occupational Therapy, Speech and
Language Therapy, Rehabilitation, Laboratory Medicine, Radiology and Outpatients services. A number of outpatient
clinics are supported by sessions taken by external consultants in Thoracic Surgery, Neurology and Oncology.
Therapy services are provided both in hospital and community settings.
Radiology provides a comprehensive diagnostic and interventional service to the Trust and, local GP and dental
practices from bases at Stepping Hill and Cherry Tree Hospitals. The MR Scanning service is provided in a new
purpose built on site unit.

The Clinical Directorates are supported by a number of non-clinical services:

Facilities Directorate – Risk Management, Controls Assurance, Security, Linen Services, Telecommunications, the
Hospital Sterilisation and Disinfection Unit, Catering, Receipt and Distribution, General Office, Hospital Cashier,
Administration Services, Car Parking Services, Voluntary Services, Transport Services, Gardening Services, Residential
Accommodation, Hotel Services Quality Control, Portering and Postal Services and Estates.

Central Management Services - Human Resources, Information, Finance and Planning. Within the Human
Resources Department is a Medical Personnel function, which provides a comprehensive service to all grades of
Medical Staff.

A purpose built multidisciplinary Education Centre opened on the main hospital site in 1994 and accommodates the
Postgraduate Centre, the Training and Development Centre, the University Of Manchester School Of Nursing and the
Education Centre Library.

Rehabilitation of Offenders Act 1974

Because of the nature of the work, this post may be exempt from the provisions of Section 4(2) of the Rehabilitation
of Offenders Act 1974 (Exceptions) Order 1975. Under these circumstances applicants for the post are not entitled
to withhold information about convictions which for other purposes are "spent" under the provisions of the Act and
in the event of employment, any failure to disclose such convictions may result in disciplinary action or dismissal by
the Trust.

Disclosure of Criminal Background – Disclosure and Barring Service

The Disclosure and Barring Service is an executive agency of the Home Office which exercises the powers and fulfils
the responsibilities of the Secretary of State under Part V of the Police Act 1997. In order to minimise any possible risk
of abuse to children or vulnerable adults or if a post is a position of trust a check will be made with the Disclosure and
Barring Service on the possible criminal background of the successful candidate for the post.

There are two levels of Disclosure service checks initially – Standard and Enhanced.

A Standard Disclosure check is primarily for: “Any employment or other work which is concerned with the provision of
health services and which is of such a kind as to enable the holder of that employment or the person engaged in that
work to have access to persons in receipt of such services in the course of his normal duties’. This check may also be
used for occupations that involve positions of trust. The Disclosure and Barring Service are authorised to disclose in
confidence to Stockport NHS Foundation Trust’s nominated officer details of any criminal record, including cautions,
reprimands, final warnings, current and 'spent' convictions held on the Police National Computer. It will also contain
information held on the Government Department Lists.

An Enhanced Disclosure check will be undertaken for a post, which involves a greater degree of contact with
children or vulnerable adults.

Any information disclosed will be treated in the strictest confidence and all circumstances will be taken into account
before any decision is reached. The successful candidate will be asked to complete a Disclosure Application Form for
a Disclosure and Barring check to be carried out, although applicants should be aware that a refusal to comply with
this procedure may prevent further consideration for the post.

Please note that for new starters to the Trust, and those moving to new posts within the Trust who will be carrying
out Regulated or Controlled activity under the ISA Guidelines there will be a requirement to check their names

To assist the Trust in reducing healthcare acquired infections (HCAI’s) all staff should be familiar with all the Trust’s
Infection Prevention policies which are appropriate to their role. You are required to attend mandatory training in
Infection Prevention and be compliant with all measures known to be effective in reducing HCAI’s”

Safeguarding

All Stockport Foundation Trust employees are required to act in such a way that at all times safeguards and promotes
the health and well-being of children, young people and vulnerable adults. Familiarisation with and adherence to the
policies and procedures relating to child protection and safeguarding of children, young people and vulnerable adults
are adhered to. It is the responsibility of all staff to report any concerns and they must therefore be aware of child
and adult safeguarding procedures and who to contact within the Trust for further help and guidance. It is an
essential requirement of all employees as is participation in mandatory safeguarding training in accordance with the

Prevention

To actively work with patients/clients/service users to identify appropriate opportunities in Making Every Contact
Count for preventative interventions which may reduce the risk of future harm to health and wellbeing and to
provide brief advice and refer or signpost to sources of further information and support which may include advice on
lifestyle behaviour and social care needs as well as safety and management of long term conditions.

Data Protection, Confidentiality and Information Governance

The post holder must abide by all relevant Trust and departmental policies including information governance,
confidentiality and data protection and, undertake the annual data security awareness mandatory training. The post
holder is reminded that any breach of the Trust’s information governance and security policies and procedures,
including data protection legislation, will result in disciplinary action.

Data Protection Legislation – the post holder is required to process all personal data relating to patients and staff,
whether in paper, electronic or other media, in accordance with the Data Protection Act 2018 and the General Data

Health & Safety

Under the Health and Safety at Work Act 1974, the Trust has a statutory responsibility to provide and maintain a
healthy and safe environment for its staff to work in. Equally the post holder is required to work within the Trust
Health & Safety Policy and fulfil a proactive role towards the management and control of risk. This entails the
identification, assessment and the immediate reporting, using the Trust Incident Reporting System, any incident,
accident, hazard or near miss involving patients, service users, carers, staff, contractors or members of the public.
