CRITERIA Good Fair Poor Justification

Onset of the illness / The onset of the patient’s illness was


on February 5, 2025. He first
experienced abdominal pain, which
he assumed was a minor discomfort
and did not pay much attention to.
However, the pain persisted
throughout the day and worsened by
the early morning of February 6,
2025.

Duration of the illness / The symptoms continued even after


his child took him for a check-up in
RHU of brgy. columbio and after
consultation, the doctor advice
laboratory tests, which the patient's
family agreed to since it was free and
after waiting for a while, upon receiving
the findings the physician referred him to
the SKPH and the family decided to get
him admitted at SKPH since it was free.
And he was brought to the SKPH by an
ambulance and arrived around 9 p.m.

Perception / The patient’s perception of his


condition is positive, even though he
still feels slight discomfort around his
abdominal area due to the recent
operation. However, the patient
verbalized that compared to before
the surgery, he feels much better and
can sleep more comfortably now.

Socio-economic status / The patient typically wakes up at 5


a.m. to work on his farm, where he
takes care of bananas and corn once
or twice a week. When he is at home,
he spends his time watching
television.
Support system / The patient’s first wife is deceased,
and he is now remarried to his current
wife. His wife and daughter are the
ones who take care of him and assist
with his needs since his parents are
also deceased, and he relies solely
on his wife and daughter.

Compliance / The patient is able to follow his


treatment regimen with the help of his
wife and daughter. They assist with
cleaning him, properly caring for his
wound, and dressing it correctly. They
have also learned how to change his
colostomy bag without causing
infection, following the nurse’s clear
instructions and health teachings.

CALCULATIONS:

Good: 3 x 3 = 9

Fair: 2 x 2 = 4

Poor: 1 x 1 = 1

TOTAL: 14 = 14/6 = 2.3

RANGE OF VALUE: 1.0 – 1.6 for POOR, 1.7 - 2.3 for FAIR, 2.4 – 3.0 for GOOD
Client has a GOOD prognosis as shown in the computation.
