Parental Consent

Dear Ma’am/Sir;

We are writing to inform you about an exciting research opportunity for your
child, _______________________________________, who is currently enrolled in
Grade 11 Betis National High School. As part of their academic development,
your child will be conducting a research project after class hours to deepen
their understanding of Practical Research 1.

The research activities will take place outside of regular school hours, as
determined by the teacher, and may involve additional work or study
sessions. We assure you that all activities will be conducted in accordance
with the school’s safety protocols.

We kindly request your consent for your child to participate in this after-class
research activity in order to collect data from _____________________________.

and I give permission for my child to participate in the research activities
outlined above, conducted outside of regular school hours.

I understand that the school will not be liable to any circumstances that
might happen outside school premises, however, the students will be
properly guided by their research adviser on the safety protocols in data
collection.

I understand that the students will only collect data in their respective
research locale outside the school, but data processing will be conducted in
the school premises with the supervision of research adviser.

I understand that the students were not required to do research activities at


their respective homes and they are reprimanded from doing overnight
sessions.

I also understand that my child will follow all guidelines and rules set forth
by the school for their safety and well-being.
