In partial fulfillment of our requirements for Action Research, we are college students of Pangasinan State
University Sta. Maria Campus taking up a Bachelor of Elementary Education major in Enhanced General
Education would like to ask your permission to allow us to conduct a survey among the Grade 6 students
in your school. This is in view of our Research entitled “Effects of Activity-Based Teaching-Learning
in Improving the Spelling Ability of Grade VI Emerald at Umingan Central Elementary School” We
will conduct a survey in a total of 25 students. Attached here with is the survey questionnaire for this
study.

Participation in the survey is entirely voluntary and there are no known or anticipated risks to
participation in this study. All information provided will be kept in utmost confidentiality and will only
use for academic purposes. The names of the respondents and the name of your school will not appear in
any thesis or publications resulting from this study unless agreed to.

After the data has been analyzed, you will receive a copy of the executive summary. If you would be
interested in greater detail, an electronic copy (e.g., PDF) of the entire thesis can be made available to
you.

If you agree, kindly sign below acknowledging your consent and permission for us to conduct this
study/survey at your school.
