I also expressed my willingness to pay the amount of ONE THOUSAND FIVE HUNDRED FIFTY PESOS (PhP1,550.00) for
his/her Field Trip fee.
I understand that the school authorities have employed all the necessary precautions in ensuring the safety of my ward
during the said activity and does not hereby release or hold the school and its officials free from any liability for any untoward
incident that shall involve my ward during field trip provided that, such incident is really foreseeable or known or attributable to the
school.
