AFFIDAVIT OF GUARDIANSHIP
I, NORLY ARSAD PUGON, of legal age, Filipino, and presently residing at
Brgy. Brgy. Gogognan, Aborlan, Palawan, Philippines after having been duly sworn
to in accordance with law, depose and state:
1. That I am the guardian/sister of NEVY R. JIMLANI, who was born on January
05, 2012;
2. That when he was about 3 yrs old our parents died and I took him in my
custody;
3. That under my custody, I took care of him, provide him food, shelter,
clothing, education and all he needs as parents would do to their children;
4. That up to this present day, he is still under my parental care, and I continue
to exercise guardianship upon him;
5. That I am fully committed to guiding and supporting the above-named
minor; and
6. That I am executing this affidavit to attest to the truthfulness of the
foregoing statements and for whatever legal purposes it may serve.

residing at Brgy. Inagawan-Sub, Puerto Princesa City, Philippines after having been
duly sworn to in accordance with law, depose and state:
1. That I am the mother of SARAH JANE V. MARIN, who was born on March
24, 2004, 2010;
2. That since I gave birth to her, I took care of her under my custody;
3. That under my custody, I took care of her, provide her food, shelter,
clothing, education and all she needs;
4. That up to this present day, she is still under my parental care, and I
continue to exercise guardianship upon her;
5. That I am fully committed to guiding and supporting the above-named
minor; and

1. That I am the guardian/grandmother of RAZIEKEY FORIO TUMALAC who
was born on February 25, 2009, at Puerto Princesa City, Philippines;
2. That when her mother left to work outside in the Philippines she left her to
my custody since she was 6 moths old;
3. That under my custody, I took care of her, provide her food, shelter,
clothing, education and all their needs as parents would do to their children;
4. That up to this present day, she is still under my parental care, and I
continue to exercise guardianship upon her;
5. That I am fully committed to guiding and supporting the above-named
minors; and
