AFFIDAVIT OF SOLO PARENT

I, RIZANEN G. REGIDOR, of legal age, Filipino, widow and a resident


of Purok 1 Centro, Cabancalan, Sevilla, Bohol, after having been sworn
to in accordance with law, hereby depose and say that:

1. That I was married to EMELIO S. REGIDOR on 16 February


2017. Out of said marriage, I gave birth to ASHLEY G. REGIDOR
on 24 May 2015 and ELLYZA G. REGIDOR on 05 February 2017.
However, my husband died on 14 October 2022;

2. Since the death of my husband up to now, I am the one who is in


custody of our children and supported them both physically and
mentally;

3. That I am executing this affidavit to declare that I am not


cohabiting with another partner or co-parent and have the sole
parental care and support of my children for the application for
benefits, legal matters; and

4. That I am executing this Affidavit to attest to the truth and


veracity of the foregoing set of facts without any promise or
reward or any other monetary consideration but solely for the
purpose of declaring the above-mentioned facts and other legal
intents.
