Surat-395007, Gujarat, India, do hereby solemnly affirm and declare as under:

1. That, I the above-named deponent do hereby declare to sponsor my


daughter’s son MAITIK JAGDISH PATOLIYA for his study in NEW YORK
INSTITUTE OF TECHNOLOGY, USA for the academic course of B.S in
Computer Science (Bachelor of Computer Science) starting from Fall 2024
for a period of 4 years. I shall bear the entire financial expenses in USA for
the required duration of the course.
