AFFIDAVIT OF NO HOMEOWNER’S ASSOCIATION

The undersigned (“Affiant”) who, after being by me first duly sworn, depose(s) and say(s) that:

2. That I am aware that a Declaration of Covenants and Restrictions are filed in the Public Records of
________________County, Florida, at Official Records Book ________, Page __________ which may
establish payment of a private, assessment.

3. That I have never paid or been required to pay homeowner association assessments, dues and/or
fees including special assessment or any other payments and are not aware of an existing
homeowner’s association.

4. That I am familiar with the nature of an oath; and with the penalties as provided by the laws of the
State aforesaid for falsely swearing to statements made in an instrument of this nature. I further certify
that we have read the full facts of this Affidavit and understand its context.

5. This affidavit is given for the purpose of clearing any possible question or objection to the title to
the above referenced property and, for the purpose of inducing ______________________ and to
issue title insurance on the subject property, with the knowledge that said title companies are relying
upon the statements set forth herein. Affiant hereby holds _______________________________ and
WFG National Title Insurance Company harmless and fully indemnifies same (including but not limited
to attorneys' fees, whether suit be brought or not, and at trial and all appellate levels, and court costs
and other litigation expenses) with respect to the matters set forth herein.
