STUDENT'S LEARNING NEEDS, INTERESTS, AND STYLE QUESTIONNAIRE

To the Respondents:
This survey questionnaire aims to gather information about your profile, learning needs, interests, and
styles which are significant in the flexible learning modality. The results will be utilized for future instructional
designing in the context of flexible learning.

You are earnestly requested to take your time in answering as honestly and accurately as possible all
the questions being asked in this survey questionnaire in order to come up with an accurate and extensive
analysis of this survey. Rest assured that all the information you share in this survey questionnaire will be treated
