Dear Sir/Madam,
I am writing to request your assistance in rectifying a mistake in my son's original birth certificate. My son's name is correctly spelled as
(Laayik Aafiith), but it was incorrectly recorded as (Laayik Aafith) on his original birth certificate.

I previously visited the relevant office to address this error and completed the necessary procedures for the correction. The hospital registrar
corrected this error in the log, which was given by them to us. However, despite this, the change has not yet been reflected in my son's original
birth certificate. This error is causing inconvenience in various official matters when we need the original birth certificate. I would be grateful if
you could expedite the process of correcting my son's name on the original birth certificate.

To assist with this, I can provide copies of the school documents of my son, my son's original birth certificate, and my son's child health record
( ).

Please advise on any further documentation or procedures required from my end to resolve this issue at the earliest.
