l, MA. THERESA E. RAMOS, of legal age, Filipino, with residence and postal address at Block 1
Lot 5 Phase 6 Roque 1, Barangay Pasong Tamo, Quezon City, after having been duly sworn to in
accordance with law, do hereby depose and says:

1. That I am the legitimate mother of the following children namely:


Ma. Christine R. Indog Date of Birth : August 20, 2012, Quezon City, Philippines
Ma. Cathryn Ramos Date of Birth : August 19, 2019, Quezon City, Philippines
2. That the aforesaid children are under my care and custody since their birth;

3. That their biological father left me sometime and I have not heard from him nor his whereabouts
for almost three years and I have remained to be the sole custodian of our children;

4. That I stand as the sole parent to my children giving them love, care, guidance and providing their
basic necessities such as food, clothing, medicine, etc.;

5. That I execute the foregoing affidavit in order to attest the truth of the above facts for whatever
legal intents and purposes it may serve.
