1. Reference: Verbal Instruction from PLTCOL JOHN JOEL S


BATUSBATUSAN, Chief of Police.

2. In connection with the above reference, you are directed to explain in writing
within 24hours upon receipt thereof why no disciplinary shall be taken against you for
not responding to the Deputy Chief of Police urgent call and for not giving immediate
feedback to the instructions given thru short message service (SMS).

3. Submit your explanation in affidavit form and personally hand-in to the Chief,
Investigation.

4. For guidance and strict compliance.
