STATUTORY DECLARATION OF AGE
I, BEN CALEB NWACHUKWU, adult, Male, LEGAL PRACTIONER, Christian, citizen of
the Federal Republic of Nigeria, of 24 CHURCH AVENUE, RUMUAPU RUKPOKWU ,
PORT HARCOURT , Obio/Akpor Local Government Area in Rivers State, do hereby
make oath and declare as follows:
1.That I am the deponent herein and make this affidavit in good faith.
2. That CHIMA BEN PRINCE is my Cousin.

3. That the said CHIMA BEN PRINCE was born on the 7 Day of September, 1997 at
ULA-IKATA.
4. That at the time CHIMA BEN PRINCE was born, a birth certificate was duly issued but
same has been misplaced and all efforts to trace same has proved abortive.
5. That I was reliably informed by his parents and I verily believe them that he was
born on the 7th day of September 1997 in ULA-IKATA Town in Ahoada East Local
Government Area of Rivers State.
6. That this affidavit is now made for record purpose and in lieu of his Birth Certificate.
That I make this solemn declaration conscientiously believing same to be true and correct by
the provisions of the Oaths ACT, 2004.
