Note:-

1. Procedure of working shall be followed on the basis of distance from track center as given in “PROCEDURE ORDER
FOR ENSURING SAFETY AT WORK SITES issued by PCSO/CR”
2. With respect to safety, proper training to contractor’s supervisors, operators, drivers and and other staff has been given
& Id cards and competency certificates have been issued, in compliance with CE Circular 190.
3. Machineries deployed total in numbers along with type and function to be mentioned. Further the, EWP shall be
periodically given in order to account for changing machineries.
With reference to the above, the work permit is hereby issued for t h e specified date, subject to the following conditions: -
Pre-Work Safety Compliance (To be verified before commencing the work):
1. Cable route marking with lime and wooden pegs done.
2. Cross trenching conducted at necessary intervals to verify the cable route.
3. Manual shifting of multiple cables executed as needed.
4. Execution of work is permitted only after the erection of safety fencing at the work site near the track.
5. SANNVERSE-ALTIS JV site engineers, safety marshals, and lookouts are assigned for safe operation.
6. Competency certifications for machine operators have been issued by Railways.
7. Details of SANNVERSE-ALTIS JV site engineers and representatives provided.
8. Joint inspection with open line S&T staff and SANNVERSE-ALTIS JV engineers completed.

Additional Safety Instructions (To be followed during work execution): -

1. Excavation work must only be carried out during daylight hours.
2. Strict adherence to worksite safety procedures is mandatory.
3. Safety teams and site engineers are instructed to halt any unauthorized work and report it immediately.
4. All site personnel, including machine operators, are briefed on the cable route plan and safety concerns.
5. Machine operations are permitted only in the presence of S&T staff, Safety staff and site engineers with valid
permissions and certifications.
6. This work permit is valid only for the specified date.

This work permit is issued for the planned Earth Work Cutting & Filling on date: 30-03-2025, subject to compliance with all
safety guidelines and work permit issued for the section and instructed by the railways on time to time.
