OWNERSHIP AFFIDAVIT
UNITED STATES EQUESTRIAN FEDERATION
The Ownership Affidavit is used to request the transfer of ownership when neither the original Lifetime or Annual Recording Certificate nor Bill of Sale is available, or when
signatures of all owners are not on the Bill of Sale.
Please complete the affidavit, have it notarized and return it to USEF with your proof of ownership (i.e.: a Bill of Sale, cancelled check, donation papers, or breed registry
papers). USEF will review your request and advise you if any further information is necessary. If the Affidavit does not explain the lack of signatures of all recorded sellers,
the transfer cannot be processed without additional proof.
