AFFIDAVIT OF LOSS

I, MARC MABAYO WABE, of legal age, Filipino and with residence address at
Purok 2A, South Poblacion, Maramag, Bukidnon, after having been duly sworn to in
accordance with law, do hereby depose and say THAT:

I was issued a Driver’s License with ID No. K10-23-000671 by LTO Maramag,


Bukidnon and Passport document with no. P8804789B issued at Cagayan De
Oro City;

Moreover, I owned a TM sim card bearing a registered number 09061567163


and inserted the same in my cellular phone;

That said Driver’s license, Passport, and cellular phone with the above-
mentioned card were placed inside the drawer of our cabinet inside our room, in
our house located at Purok 2A, South Poblacion, Maramag, Bukidnon;

Lately, our said house burned down where the said Driver’s license, Passport,
and cellular phone with the above-mentioned card were kept;

That until now, despite earnest effort to locate, the said Driver’s license,
Passport, and cellular phone with the above-mentioned card, are still nowhere to
be found such that I now believe that they are now lost and beyond recovery;

That the said Driver’s license, Passport, and cellular phone with the above-
mentioned card have not been confiscated by any officer of the law or the issuing
office or agency due to violation of any law, statute, order, rule or regulation;

As such, I am executing this affidavit of loss to attest the truth of the foregoing
and for whatever legal purpose this affidavit may serve.

FURTHER AFFIANT SAYETH NONE.
