I fully understand that:
1. I am temporarily enrolled.
2. I cannot be issued Transfer Credentials – True Copy of Grades and Certificate of Good
Moral Character.
3. I cannot enroll in the next semester.
4. I will be bounded to enroll at Pamantasan ng Cabuyao once I gave my consent to the
Office of the University Registrar, Pamantasan ng Cabuyao to register me.
4.1.1 Further, I understand that I have to comply on the scheduled aptitude test for
collegiate students to be admitted in the board programs of the University.
5. If I am a transferee [refers to students who have attended other schools duly recognized
by DepEd or CHED] / returning student [refers to students who have been on Leave of
Absence (LOA) or Absence Without Official Leave (AWOL) and may apply for re-
admission to the university] / second degree taker [refers to the bachelor’s degree
holder who wish to take a second-degree program], all my earned units of general
education courses with passing grade from my previous school may or may not be
credited depending on the curriculum assessment to be evaluated by the Program Chair
or College Dean, and for endorsement to the University Registrar for official crediting of
subjects. All professional courses must be taken in PnC-UC.
5.1.1 Further, I understand that I may not be covered or qualified by the UniFAST
program. Therefore, I will pay for my tuition, miscellaneous and other fees relative
to my enrolled program.
With all the foregoing, I shall hold free from any liability, whether civil, criminal,
administrative, the University personnel who are involved in my acceptance and enrolment,
and the enforcement of any law or rule and the obligations provided in this document.
I hereby agree that this affidavit is executed to attest to all the above conditions in
compliance with the requirements of Pamantasan ng Cabuyao as basis for the approval of
my application for enrollment this semester.
