I, DIONICIO AGBUNAG BALA, of legal age, married, Filipino Citizen and resident of Dadap,
Luna, Isabela, Philippines, do hereby name, constitute and appoint, ANN DEBBIE BALA CORPUZ, of
legal age, single, Filipino Citizen and also a resident of Dadap, Luna, Isabela, Philippines, to be my true
and lawful Attorney-in-Fact for me, in my name, place and stead, to do and perform the following acts, to
wit—
1. To transact, request and claim/ receive on my behalf my Certificate of Live Birth before
the office of Philippine Statistics Authority;

2. To sign all papers necessary to accomplish the foregoing transactions/tasks as well as to


do and perform all acts and things necessary in order to accomplish the foregoing tasks.

HEREBY GIVING AND GRANTING unto my said Attorney-in-Fact full powers and authority to
do and perform the foregoing tasks as if I was personally present and hereby confirming and ratifying all
that my attorney-in fact shall lawfully do or cause to be done by virtue of these presents.
