Mental health disorders have become a global crisis affecting millions of
individuals across diverse demographics and cultures. The World Health
Organization (WHO) reports that approximately one in eight people
worldwide suffer from mental health disorders, with depression being
particularly prevalent, affecting over 264 million individuals, especially
those aged 15-28 years. The COVID-19 pandemic has exacerbated this
crisis, leading to increased stress, anxiety, and depression across all age
groups.
In India, the situation is particularly concerning, with the National Institute
of Mental Health and Neurosciences (NIMHANS) reporting that 14.7% of
the population experiences some form of mental illness. Chronic stress,
which is a significant contributor to mental health issues, manifests as
various physical, emotional, and cognitive symptoms. These include
elevated heart rate, weakened immune system, anxiety, irritability,
impaired judgment, and reduced concentration. Prolonged exposure to
stress can lead to serious health complications such as cardiovascular
diseases, diabetes, and gastrointestinal disorders, as well as an increased
likelihood of substance abuse and reduced workplace productivity.
Despite the widespread need for mental health support, access to
professional assistance remains limited. The WHO recommends one
therapist per 10,000 individuals; however, in India, the ratio is alarmingly

sophistication, rendering them valuable tools across various domains
including healthcare and mental health support.
AI chatbots have gained prominence in mental health care because of their
capacity to engage in natural-language dialogue with users. These chatbots
have evolved since their initial development, and are now extensively
integrated into mental health interventions. They enable seamless
communication, while eliminating barriers related to time and location,
thereby making therapy more accessible to individuals who may otherwise
face challenges in accessing traditional mental health services. AI-powered
therapy chatbots utilize extensive datasets to develop predictive models and
personalize interventions, thereby enhancing their efficacy in addressing
mental health concerns.
AI chatbots offer numerous advantages for mental health. Research
indicates that over 55% of students use AI-powered chatbots for emotional
support, guidance, and coping strategies. The anonymity afforded by
chatbots encourages users to seek assistance without fear of judgment,
whereas their non-judgmental and readily available nature ensures their
continuous support. Many individuals prefer AI chatbots to human
therapists for initial therapy sessions because they provide a comfortable
and accessible medium for discussing mental health concerns.
Several studies have demonstrated the efficacy of AI chatbots in mental
healthcare. Woebot, an AI-based cognitive-behavioral therapy (CBT)
chatbot, was found to significantly reduce depressive symptoms within two
weeks (Fitzpatrick et al., 2017). Wysa, another AI-powered mental health
chatbot, provides meaningful emotional support and improves users’ well-
being (Inkster et al., 2018). Replika fosters trust and companionship,
whereas Tess assists users in self-guided mental health interventions
(Fulmer et al., 2018). These studies underscore the accessibility, scalability,
and effectiveness of AI-driven mental-health solutions. However, concerns
persist regarding user trust, data privacy, and chatbots’ limitations when
managing complex psychological issues.
The global mental health crisis highlights the need for accessible and
effective interventions. Nearly one billion individuals worldwide experience
mental health issues, and by the age of 75, approximately 50% of
individuals are likely to develop mental disorders. Despite the increasing
prevalence of mental health problems, access to professional treatment
remains inadequate. In high-income countries, only 23% of individuals with
depression receive appropriate care, whereas this figure drops to 3% in low-
income countries. AI-driven chatbots present a promising solution by
bridging this gap and providing scalable technology-based mental health
support.

according to predefined rules and are incapable of processing open-ended
