side windows clean and free of obstruction / dirt
including screens?
7 Is the cabin free of unwanted materials which can
influence the operation of control levels and or

9 Is there any evidence of regular maintenance of
machinery?
10 Lights, brakes, horns, alarms, ROPS, rotating amber
beacons light and all safety devices working?
11 Seat belt provided and used?
12 Mirrors provided in good condition and correctly
focused?
13 Is the equipment used for the purpose it is designed
for?
14 Is there a banksman assigned for the machinery?
15 Is fire extinguisher available?
16 Is there considerable air pollution due to exhaust
emission (visual)
17 Lock starting system when not in use
18 Other (please specify)
