AFFIDAVIT OF LOSS
I, ANGELICA A. UTBO, Filipino, of legal age, single and with residence at Purok 6,
Poblacion, New Corella, Davao del Norte, Philippines, after being duly sworn to in accordance
with law, hereby depose and state that:

1. I was an alumna and a holder of a Senior High Student Card issued by New Corella
National High School.

2. The Card Contains my Grades and other personal information.

3. I was keeping the said card years ago after I graduated and recently discovered that it was
no longer in my stash of documents.

4. Notwithstanding the efforts exerted to locate the above said item, it can no longer be
found and may now be considered irretrievably lost; and

5. I am executing this Affidavit to attest to the truth of the foregoing facts and to appraise
the authority concerned and for whatever legal purpose it may serve.
