Safety Officer Inspection Checklist – Deck & Structures
This Checklist is to be completed quarterly in February, May, August and November.
Inspection should be carried out by the following members of ship’s crew: the Master, the Chief Officer, the Chief Engineer and the Bosun.
Following areas should be inspected: Weather Decks, including Decks around the Superstructure, Forecastle, Pumproom (if available), Fire Stations, CO2 Station,
Foam Station, Cargo Holds, Cranes, Stores Lockers.
The Safety Inspection shall aim to identify hazards and potential hazards to health, safety and the environment. The inspection shall include all accessible areas of
the vessel. All findings shall be recorded and discussed during Monthly Safety Committee Meeting.
When hazards are identified that cannot be rectified by the vessel personnel, then the Company shall be informed, AMOS INR – ‘Hazard Identification’ in order for
remedial action to be taken.
Ship Structures YES NO
Hull, deck fittings & equipment and pipe lines on weather decks free of apparent corrosion, pitting, cracking, deformation, fracture and
leakage.
Guardrails, catwalks and bulwarks without corrosion, buckling and fractures.
Weathertight doors and access hatches free of heavy corrosion and deformations.
Closing appliances of weathertight & watertight doors and access hatches (incl. gaskets and locking devices, etc.) not seized or damaged.
Self-closing devices and/or securing devices (where fitted) of weathertight , watertight & fire doors and access hatches in working condition
and prevent doors or hatch lids against movement or accidental closing.
Ventilators and air pipes in satisfactory condition; no corrosion, holes and deformations; floats in the heads are free and operational; wire
gauzes fitted to fuel oil tanks air pipes in satisfactory condition.
Hydraulic systems on deck in good condition without oil leakage.
Fire flaps and dampers are not seized; closing appliances, and gaskets are in order; flaps/dampers’ marking available as per closing down
plan.

Accommodation ladders and gangways and their steps kept in good condition; wires not rusty and properly lubricated; marking plate
available on both ends of ladder.
Ladders outside and inside the accommodation block and their steps kept in good condition and escape routes well lit and not obstructed.
IMO symbols suitably posted.
Means of escape and fire exits clearly identified and unobstructed.
Stairs and exterior areas well enough lit to enable users to move about safely.
All stairways, elevated platforms, etc. unobstructed.
All fixtures and fittings over which seamen might trip or which project, particularly overhead, thereby causing potential hazards, suitably
painted or marked.
Ventilation and air conditioning air intakes grids clean and in satisfactory condition.
Handrails without corrosion, buckling and fractures.
Pilot ladders/hoist and arrangement for safe boarding, safety net and lights in good condition.
Crew working in the area wearing all necessary protective clothing and equipment that is in good condition and being correctly used.
Permits-to-work used when necessary.
Lock out/Tag out procedure is used as appropriate.
Stretcher (or Safety harnesses, if stretcher cannot be safely used) and lanyards available, inspected as required and ready for use.
Stretcher or Safety harness in pump room (where applicable) and lanyards available, inspected as required and ready for use.
All ship compartments safe to enter.

Any gear, stowed within the ship compartments, suitably secured.
All guard-rails in place, secure and in good condition.
All gratings well secured, all loosened grating fixtures tightened
All openings through which a person could fall, suitably fenced.
When portable ladders are in use, they properly secured and located at a safe angle.
In case of using the mechanical equipment used for lifting and manual handling, the staff is trained in its use, and there is enough space to use
it.
Where mechanical assistance is not possible for lifting and manual handling, the staff trained in safe lifting techniques and uses these
techniques.
All Spill Containment Arrangements for cargo / slop tanks manifolds, bunker manifolds, bunker tank vents and deck hydraulic machineries
have been inspected and tested including its drain plugs, if provided, and found effective.
The height of any savealls around bunker and lubricating oil tank vents are not greater than the vent heads themselves, because this could lead
to the ingress of water in bad weather if the savealls become filled with water. The vent heads are clearly labelled to indicate the space that
they serve. Containers are kept empty of water and free of oil. Drain plugs are in place, visually inspected for signs of corrosion and to ensure
they remains effective and not leaking when plugged.
Arrangements for the removal of cargo and oily water accumulations from manifold drip trays are inspected to ensure their effective
operational condition.
Containment fitted around hydraulic and other deck machineries are visually inspected for signs of corrosion and to ensure that drain plugs
are remain effective with no loose when they are secured.
All main deck scuppers have been inspected and tested. The tightness test and inspection included leak test with water and physical
verification.

Cargo Spaces (Dry-Cargo Vessels & Bulk Carriers)
Cargo hold structures, lashing fittings & equipment and pipe lines in cargo holds and cargo decks free of apparent corrosion, pitting, cracking,
deformation, fractures and leakage.
Hatch covers are in operational condition and can be properly secured immediately after closing or opening.
All locking and tightening devices operational; securing cleats kept greased.
Hydraulic systems on deck for hatch covers in good condition without oil leakage.
Entry to holds/cargo spaces is arranged through the permanent means of access and being kept in good condition.
Access doors and hatches to cargo holds free of heavy corrosion and deformations; closing appliances (incl. gaskets and locking devices, etc.)
not sized or damaged.
Ventilators in cargo holds in satisfactory condition; no corrosion, holes and deformations; grids intact and free of apparent corrosion, pitting,
cracking, deformation, fractures.
Wooden covers or steel pontoons where fitted together with associated portable beams, carriers and sockets for the portable beams and their
securing devices (incl. tarpaulins, cleats,
battens and wedges, etc.) in good working condition.
Guardrails, catwalks and bulwarks without corrosion, buckling and fractures
Cargo holds coamings in satisfactory condition (incl. gaskets, gaskets lips, compression bars, drainage channels, etc.)
Pipelines located in cargo holds free of apparent corrosion, pitting, cracking, deformation, fracture and leakage.
Loose gear, strops and lifting equipment stored properly and tested & marked as required.
Means of escape and fire exits clearly identified and unobstructed.
Electrical safety
Lighting levels adequate.

Emergency lighting provided and tested regularly.
Accommodation spaces, working spaces and corridors are properly lit;
Light shades and diffusers clean and in good order
All electrical equipment, fittings, and tools regularly checked and maintained.
No unauthorised electrical equipment in workshop or other compartments.
Electrical cable connectors are maintained duly isolated to avoid risks of electrical shocks; no loose wiring.
Portable tools grounded or doubled insulated
Lighting in workshops, stores lockers, emergency stations, etc. adequate for the task.
Life-Saving Appliances
Lifeboats and rescue boat are structurally sound and properly stowed, engines start without difficulty.
Lifeboats and rescue boat inventory and ration adequate and in good condition.
Lifeboats and rescue boat davits (incl. wire falls) well maintained, release mechanisms in working order and in correct position, retro-
reflective tapes provided, annual & 5-year thorough examinations done.
Fall Preventer Devices are fitted to lifeboat(s) and rescue boat, where applicable, properly marked and in good condition.
Liferafts and hydrostatic release units serviced, liferafts properly stowed, hydrostatic releases properly connected.
Liferafts launching arrangement (if fitted) in good working condition.
Sufficient lifebuoys provided and marked, smoke/lights and lifelines are in good working condition, quick release mechanisms are working.
Sufficient lifejackets and immersion suits are provided and marked, lights and whistles are attached and in working condition, retro-reflective
tapes provided.

Means of embarkation for lifeboats and liferafts readily available, emergency lights at
survival stations are working, overside lights in good working condition.
Donning instructions, launching instructions and IMO symbols are suitably posted.
LSA manufacturer’s maintenance instructions and SOLAS training manuals available.
Fire Fighting Appliances
Fire escape routes clearly signed, kept clear and wide enough to prevent a crush, and do they lead quickly and directly to a safe area.
Fire dampers/ventilators not seized and clearly marked; skylights in good condition.
All flammable liquids stored appropriately and working stocks minimized.
Pollution Prevention
Garbage reception facilities adequate, categorised and properly marked; warning placards against prohibited discharges posted.
General waste, packing and other unwanted material removed promptly to prevent it accumulating.
All hazardous substances/wastes segregated, stored (and locked, where necessary) in suitably labelled containers and locations pending
disposal.
