Ankle Fracture Management System
Introduction
Ankle fractures are frequently complicated by variables out of the surgeon’s control.
The amount of comminution, degree of osteoporosis, location of the fracture, and size
of the patient can all lead to challenging obstacles. The Ankle Fracture Management
System was developed to be the most comprehensive set available for the treatment
of common injuries. Improved basic small fragment implants and instrumentation,
anatomic plates, and locking-screw technology, allow for confident and effective
treatment of the majority of fracture patterns.

This set was designed to give the surgeon the armamentarium to more effectively
address any patient/injury pattern encountered. Significant time and research were
invested to develop anatomic plates, with specific emphasis placed on determining the
appropriate angle of the locked screws. This allows for easier and faster implantation,
while negating the need for more expensive “variable-angle” locking plates and screws.

Included in the set are a variety of fracture-specific plates engineered to address


even the most difficult fracture patterns. These consist of Third Tubular Plates, 3.5 mm
Reconstruction Plates, Lateral Hook Plates, Medial Hook Plates, and Complex Fibular
Fracture Plates.

All plates designed for use on the fibula have modifications that allow for easier
use of either the TightRope® or syndesmotic screws for stabilization of associated
ligamentous injuries.
Complex Fibular Fracture Plates
The Complex Fibular Fracture Plates, otherwise known as the
Locking Distal Fibular Plates, are designed for use in the most
challenging lateral malleolus fractures. The distal portion or head
of the plate thins and spreads to encompass a large portion of
the lateral malleolus. The head accepts multiple 2.7 mm locking
screws or 3 mm cancellous screws, making this plate ideal
for patients with highly comminuted lateral malleolus fractures,
osteoporotic bone, obesity, or open injuries with associated bone
loss. Included are divergent angle screw holes that increase
biomechanical stability and provide for more secure fixation.
The shaft portion of the plate thickens to improve rigidity and
strength, and the multi-functional screw holes allow for multiple
screw options. As with all of the fibular plates, there are recessed
screw holes and a scalloped perimeter, which allows for easier
contouring and Knotless TightRope/screw placement outside of
the plate, as necessary.

screw as fixation. The plate holes
are nested so the round button of the
TightRope sits flush, reducing overall
profile. The scalloped design also
allows for the TightRope placement to
be posterior to the plate, if necessary.

Locking Third Tubular Plates
Low profile plates are designed for use with the majority
of fibular fractures. The scalloped design allows for easier
contouring and placement of TightRope/screws outside of
the plate, when necessary. The arch maintains strength while
decreasing surface contact with the periosteum, preserving
blood supply. All screw holes accept 3.5 mm nonlocking
cortical, 3.5 mm locking, or 4 mm cancellous screws. Nested
slots at the screw holes lessen screw head prominence and
allow the TightRope’s circular button to fit snugly.

Reconstruction Plates
The Locking Straight Plates are thicker and stronger than
Locking Third Tubular or Locking Distal Fibular Plates. They are
designed for use with more comminuted fibular shaft fractures,
larger athletes, and obese patients. The scalloped design
allows for easier contouring. Like the Third Tubular Plates, all
recessed screw holes accept locking or nonlocking screws,
reducing screw head prominence.

Syndesmosis Fixation Options:


Fig 2, 2a and 2b – If syndesmosis
stabilization is required, it is important
to appropriately angle the fixation device
chosen (TightRope or screw), ensuring
proper reduction of the syndesmotic
joint. This typically requires about 30º
of anterior angulation, towards a more
anteromedial position in the distal tibia.
If desired, the TightRope can be used
outside of the plate for an even lower
profile construct.

Locking Medial Hook Plates
The Locking Medial Hook Plate is designed to treat extreme
distal or comminuted medial malleolus fractures. The two-hooked,
distal prongs are designed to corporate the deltoid ligament
attachment, affording secure fixation of these small fragments (akin
to using the rotator cuff in assisting greater tuberosity fixation in
the shoulder). The prongs are spaced and recessed appropriately
to allow for placement of a 4 mm cancellous screw, if desired.
As these small fragments typically do not permit the placement of
two 4 mm screws, this plate improves the strength of fixation and
improves rotational control. The proximal screw holes allow for the
placement of 3.5 mm nonlocking cortical screws, 3.5 mm locking
screws, or 4 mm cancellous screws and 2.7 mm locking or 3.0 mm
cancellous distally.

tightening only after compression
across the fracture has been achieved.

The anatomic, low profile plate is designed to treat fibular
fractures with extreme distal extension. The plate is anatomically
designed to fit the lateral aspect of the fibula, with 2.7mm locking
screw holes in the distal portion for more reliable fixation (3 mm
nonlocking cancellous screws are also available). The two-
hooked, distal prongs are designed to incorporate the calcaneal
fibular ligament attachment (akin to using the rotator cuff in
assisting greater tuberosity fixation in the shoulder). These prongs
are spaced and recessed appropriately to allow placement of a
4 mm cancellous screw, if desired.

The Ankle Fracture Management System was designed to give
surgeons the ability to fix the majority of ankle fractures, while opening
only one set. As described earlier, the set contains five different
locking/nonlocking plates and a wide variety of screws which should
easily accommodate the most common ankle fracture cases.

Offering multiple fixation solutions, the Ankle Fracture Distal Tibia Screw
Case (pictured below) is an auxiliary container which complements and
may be added to the system to fulfill the needs of the surgeon when
considering more complex cases. The AR-8943C-31 case is currently
available and houses both solid and cannulated cortical screws,
cancellous screws, as well as a wider range of sizes of those screws
already found in the main set (AR-8943S, pictured left).
