3.3 Lessons Learnt: Fatal - 2018
Event Description:
• On Sunday 4th March 2018 at 3:30 PM, xxxx, Sub contractor worker, xxxx, 19 years old dead due to
contact with electricity while he changed working place to the next pole continue the cable tension
and Galvanize band hoop installation. The victim is standing at the pole and near the High –tension
line.

Root Causes:
•Lack of Skill : Inadequate performance and Lack of coaching
•Inadequate Leadership and/or Supervision : Lack of supervisory/management job knowledge
•Inadequate Engineering : Inadequate standards, specifications and/or design criteria

Temporary Action:
• Immediately stop all activates.

Corrective Action
• To safety training needs analysis
• Critical task procedures/practices updated
• Personal Protective Equipment Available.

3.3 Lessons Learnt: Fatal - 2019
Event Description:
• In 15-Feb-2020, 11:00 AM, the Survey Team arrived at the site and registered at the Factory reception (the site is
inside of factory) but they did not make site access request officially to tower company and enter the site without
permission from tower company.

Root Causes:
•Failure to use PPEs(fall protection safety Equipment) effectively : The Victim (Rigger) did not use Positioning
Lanyard. He wore full Bodyharness with Double Lanyards but just wearing it.
•Inadequate Leadership and/or Supervision : The Site Supervisor did not monitor/enforce to follow the HSE Rules
•Inadequate Engineering : Inadequate standards, specifications and/or design criteria (it did not install proper step
ladder on the upper part of Tower

Temporary Action:
• Immediately stop all activates.
Corrective Action
• Not to enter the site unofficially
• If the Rigger faces special cases such as No Climbing ladder, Tower/ pole body too big and cannot fix the lanyard;
stop the work immediately and then the Rigger & SV must report to escalate this cases to relevant management
team.
• Training to Riggers and SVs regarding working at heights and case study regarding the accident
• Enforce Penalty to LSP Riggers & SVs who did not follow HSE rules, 50% remote checking and 50% on site HSE
random checking by HQ HSE Team
