Nursing 150 Clinical Reflection

families.
Today I rendered care for a 24-year-old who had a chief complaint of
hydronephrosis. Patient was born with a birth defect, and the mother didn’t want to
get him admitted because she scared to lose her son. For all his 24 years living on
this earth he has always been in pain, until his left kidney was swelling to a point it
is crushing his right kidney. Patient stated he thought he was gaining weight when it
really was backed up fluid due to the dysfunction of his kidneys. Patient had a
nephrectomy, post op day 1. I had to look for signs and symptoms of infection, foul
smelling odor, purulent drainage, swelling and I also had to prioritize his airway,
breathing, circulation. I also ambulated with him twice and gave a teaching on the
use of incentive spirometer and educated him that it is used to open his airway.
Patient had crepitus on his left lower lobe. I palpated and I felt a grinding sensation.
Patient was stable and all vitals were within limits.
