• This Declaration is applicable to all HOT WORKS involving flame, hot air, arc welding and cutting
equipment, brazing and soldering (excluding the use of electric soldering irons), blowlamps,
bitumen boilers, angle-grinders and any other heat producing equipment as specified in the
Method Statement.
• Dates will be specified on “HOT WORK PERMIT” for weekly basis.
• Hot Works Areas will be inspected and verified by contractor HSE Supervisor and Facility
Management representative / supervisor.
We undersigned, confirm that we have understood the site safety rules and the restrictions on Hot Work
Permit. All hot works will be carried out in accordance with best current trade practice and that all
appropriate health and safety controls.
We confirm that:
✓ This declaration is completed today before commencing HOT WORKS and submitted to site security.
✓ I (Contractor HSE Supervisor) will be physically present onsite of HOT WORKS to supervise the team
as required throughout the time duration mentioned above.
✓ The team shall access and work only in the designated and permitted Hot work areas.
✓ The location of work has been examined and there are no flammable and combustible liquids,
vapours, gases, dusts or other materials / hazards. Area is protected against heat / ignition for up to
8 to 10 meters (around, above and below the Hot works area).
✓ For HOT Works, a fire warden will be stand by with an extinguisher appropriate to the fire risk. His
role will be that of a fireman and he will not be directly involved in the execution of any part of the
works.
✓ The operatives, including the “fire warden”, are fully aware and have been instructed of the building
fire procedures and the whereabouts of the nearest fire alarm points and telephones.
✓ Any change in the scope of work, details of circumstances and assumptions not declared
automatically revokes this declaration and permit. Work can be stopped immediately.
✓ Such changes if any will be notified to ATM/ PM FM, Security and HSE representative immediately
by email.
✓ POST WORK COMPLETION: The work area will be cleaned, inspected and restored to original
conditions.
