I am writing to formally request a leave of absence from my duties as a barangay health worker at
Mangcamagong Barangay Health Station. I have been accepted into the Ako Bicol Government
Internship Program, which is a 6-month opportunity starting on June 3. This internship will provide me
with valuable experience and skills in the field of Public Service.

Upon the completion of my internship, I am fully dedicated to returning to my position as a Barangay


Health Worker, where I will apply the skills and insights gained during my time at Ako Bicol GIP to
enhance the health care services provided to our community.
