Regular moderate exercises for 30 minutes.

occasional bloody stool with mucus. Stool culture showed cysts of

Dispense: Metronidazole – 15 tablets; Diloxanide - 30

cough with expectoration for the past one month. Detailed

history reveals that he has loss of appetite and weight, chest pain, night
sweats. He also has evening rise of temperature. Sputum examination
reveals acid fast bacilli. Chest X-Ray shows apical pulmonary
infiltrates.

Diagnosis: Pulmonary Tuberculosis

Tablet ISONIAZID 300 mg.
One tablet to be taken once daily before breakfast for two months.
Tablet RIFAMPICIN 600mg.
One tablet to be taken once daily before breakfast for two months.
Tablet ETAMBUTOL 1000 mg.
One tablet to be taken once daily before breakfast for two months.
Tablet PYRAZINAMIDE 1500 mg.
One tablet to be taken once daily before breakfast for two months.
Tablet PYRIDOXINE 10 mg.
One tablet to be taken once daily after food for two months.
Follow up after one month.

• A 55 yr old, known hypertensive male patient presented to cardiology
OPD complaining of heavy, pressing substernal pain radiating to jaw
and breathlessness. Pain increased with stressful activity and was
relieved with rest. BP was 144/96 mm Hg. ECG and Serum cardiac
enzymes were normal.

• Prepare a rational prescription for this patient.

Diagnosis: Stable Angina
Rx
Tablet NITROGLYCERINE 0.5 mg.
One tablet sublingually as when required up to three doses 5 minutes

Tablet METOPROLOL 25 mg.
One tablet to be taken once daily for 7 days.
Tablet ASPIRIN 75 mg.
One tablet to be taken once daily after food for 7 days.
Tablet ATORVASTATIN 20 mg.
One tablet to be taken once daily at night before sleep for 7 days.
