I. RATIONALE
Under Section 29 of the Universal Health Care (UHC) Act, which mandates
the identification of Unserved and Underserved areas by the Department of Health
(DOH), the Bureau of Local Health Systems Development (BLHSD) developed the
Administrative Order (AO) 2020-0023 to provide the criteria to identify the said areas.
This was further supported by releasing the profiling and information systems toolkit
disseminated through Department Memorandum 2021-0295. The said toolkit was
used for the profiling of the unserved and underserved areas and updating of its
annual list, which served as the basis for the prioritization of assistance from the
national and local governments and other health partners as mandated by Chapter
V, Section 2 of the UHC Act.
Three years after its implementation, the toolkit was subjected to a validation
study by BLHSD, which showed a variance in accuracy due to issues and difficulties
in data collection and the use or interpretation of the data. The study also highlighted
the need to review and revise AO 2020-0023 to address the gaps and challenges in
identifying and prioritizing the Unserved and Underserved Areas. To address this, the
BLHSD, in collaboration with CHD XI, is conducting a research study on prioritizing
the Unserved and Underserved Areas (UUA) and the criteria for identifying such
areas.

II. BARANGAY PROFILING PROCESS
If the submission is not approved, the process loops back to Step 2 for
corrections and resubmission. If approved, the data proceeds to the first validation,
conducted by the DOH Representative. This step involves a physical and desk
review to ensure the accuracy and reliability of the submitted data. If the data fails
validation, it undergoes review, physical validation, and correction before
resubmission. Once the data is successfully validated, it proceeds to the encoding
phase, where the verified data is officially entered into the system. This systematic
approach ensures that profiling for unserved and underserved barangays is
accurate, credible, and efficiently documented.

III. BARANGAY VALIDATION PROCESS
The Barangay Validation Process begins after completing profiling steps 1-4,
ensuring that initial barangay data is gathered. The process moves to 1st Validation,
where the DOH Representative conducts a physical and desk review to validate the
submitted data. If the data is not approved at this stage, it is sent back for review,
physical validation, or encoding correction before being re-evaluated. Once the data
passes the 1st Validation, it is labeled as validated and proceeds to encoding by the
LGUs (Step 6).
Following encoding, the process undergoes a 2nd Validation (via the Kobo
toolbox), where the barangay profiles are randomly validated by
Coordinators/BLHSD. The profiles are returned for further review and corrections if
there are discrepancies observed. If the profiles are approved, they proceed to the
approval of the barangay profile stage (Step 8). The BLHSD will generate the official
list of profiled barangays for concurrence.
Upon successful generation, the process moves to the final step, which
involves concurrence of the generated list (Step 9). If issues arise during validation,
the process loops back to Step 2 for necessary corrections. This systematic and
thorough process ensures accurate and reliable identification and documentation of
unserved and underserved areas.

IV. BARANGAY DATA GENERATION PROCESS USING THE KOBO TOOLBOX
After encoding, the process undergoes Validation, where the encoded data is
randomly reviewed to confirm its accuracy. Once validated, the system generates the
Unserved and Underserved Areas List (Step 8). This list is then submitted for
concurrence by the Centers for Health Development (CHDs) and MOH - BARMM
(Step 9) to ensure alignment with local and regional priorities.

V. UNSERVED AND UNDERSERVED AREAS PROVINCIAL PROFILING,
VALIDATION, AND DATA GENERATION PROCESS USING THE KOBO
TOOLBOX
The Provincial Data Generation Process ensures data availability to fulfill the
criteria for identifying the unserved and underserved areas. This process is initiated
by completing profiling step 1, where initial provincial data is gathered. After profiling,
the process advances to Validation Step 2, where the accuracy and reliability of the
data are confirmed. Following validation, the data undergoes encoding done by the
encoders in the LGUs (Step 4) using the Kobo toolbox. A Kobotoolbox Tutorial is
available in Section VI of this toolkit to guide users through encoding.
Post-encoding, the process enters Validation (Step 5), where the encoded
data is subjected to random evaluation to ensure the accuracy and completeness of
submissions. Upon successful validation, the provincial profile is generated to serve
as the basis for identifying unserved and underserved areas, particularly in fulfilling
the criteria on health indicators.

VI. STEP-BY-STEP ENCODING PROCESS USING THE KOBO TOOLBOX
CREATING A KOBOTOOLBOX ACCOUNT
1. Go to the KoboToolbox Sign in or create an account page:
https://www.kobotoolbox.org/sign-up
2. Click Create an account under the Global Kobotoolbox Server.
Important note: It is not possible to share projects and data
between the two servers. Make sure you create an
account in the Global Kobotoolbox Server.
3. Enter the details on the form and click CREATE ACCOUNT.
4. KoboToolbox will send you an email with a link to activate your
account. Click the activation link sent to you.
SETTING UP KOBOCOLLECT
1. Download the KoboCollect app on your Android device
2. Open KoboCollect
3. Select Manually enter project details
4. Input the URL, Username, and Password, and then select Add
5. Click on the icon at the right-hand corner of the app
6. Click Settings
7. Click Form management
a. Tick Automatic download
8. Back on the Settings page, click User and device identity

VII. DEFINITION OF TERMS
1. GENERAL INFORMATION
1.1. Purok/ Sitio/ Zone - Minor unit/ sub-divisions of barangay; Political
subdivision of a barangay. The smallest unit of governance in the
Philippines. Typically composed of twenty to fifty (20-50) households,
depending on the particular geographical location and cluster of
houses. If created and given a mandate by an ordinance of the
barangay, municipality, or city, a purok/sitio/zone could perform
government functions under the coordination and supervision of local
officials (Abinales, Patricio N.; Amoroso, Donna J. 2005).
2.2. Armed Conflict - This is usually seen as criminal acts or peace and
order problems at the local level. The experience of conflict, then,
extends beyond peace and order concerns to include poverty (conflict
over wealth and resources), gender violence, and other forms of
structural violence, e.g., Territorial disputes, ideology-based conflicts,
political killings, and clan wars (Guidebook on Conflict-Sensitive and
Peace-Promoting Local Development Planning, OPAPP, 2016).

2.4. 4Ps/ CCT/ MCCT - Pantawid Pamilya Pilipino Program(4Ps)/
Conditional Cash Transfer (CCT) is a poverty alleviation program of
DSWD. Modified Conditional Cash Transfer (MCCT) is the CCT made
for IPs and those individuals living in GIDAs. (DSWD, 2022)
3.3. Fully Immunized Child - Infants who received one dose of BCG, three
doses of OPV, three doses of DPT-HiB-HepB vaccines, and two doses
of MMR vaccine by 12 months. MMR1 at 9 months and MMR2 at 12
months.
3.4. Adolescent Live Births - Number of live births byb adolescent women
aged 10 - 19 years old
3.5. Current Contraceptive Users - Sum of the current users at the end of
the month/ quarter (Formula: New acceptor of the previous month +
Other acceptors of the present month - Drop-out of the present month)

The identification of the 2025 list of Unserved and Underserved Areas will be
subjected to a validation process done by either the Coordinator, the Development
Manager Officer (DMO)/DOH Representative, or the Provincial DOH Office as stated
in Section III and V of this toolkit. This validation will include a review of physical and
encoded data supported by documents such as a fully accomplished and signed
Barangay/Provincial Profiling tool, Barangay/Municipal/Provincial Certification, and a
copy of the Rural Health Unit or National Field Health Service Information System
(FHSIS). These attachments will serve as a means of verification to attest to the
veracity of the submitted data. See Annex C for samples of MOVs.

XI. TIMELINES FOR THE 2025 UNSERVED AND UNDERSERVED AREAS
PROFILING, VALIDATION, AND ENCODING PROCESS AND RELEASE OF
OFFICIAL LIST
From December 2024 to May 2025, local health units are primed and oriented
on the 2025 profiling tool. Starting January 2025 or after the orientation, CHDS/MOH
BARMM or LGUs will gather data on their respective barangays or provinces using
the 2025 Profiling tool. From June to August 2025, the collected data will be encoded
and submitted to regional coordinators/DOH representatives for initial validation. The
submitted data will be reviewed for accuracy and completeness before consolidating
and submitting it to the Bureau of Local Health Systems Development (BLHSD).
From July to September 2025, the BLHSD will start accepting submissions
and thoroughly review the consolidated data to ensure its quality and consistency. In
the third quarter of 2025, the 2025 List of Unserved and Underserved Areas will be
finalized and concurred upon by relevant stakeholders. The official list, released in
the fourth quarter of the current year, will then provide a comprehensive overview of
areas requiring targeted health interventions.
This systematic process will ensure that the profiles of unserved and
underserved areas are accurately identified.
